Aspergillus fumigatus je široce rozšířená saprofytická houba, která se často uplatňuje jako oportunní patogen. Způsobuje onemocnění aspergilózu, která se může projevovat i jako život ohrožující infekce u imunokompromitovaných pacientů. Azolové deriváty patří mezi léky první volby v terapii aspergilózy, ale celosvětově se zvyšující výskyt azol-rezistentních kmenů A. fumigatus představuje závažný klinický problém. Studie posledního desetiletí ukázaly, že především důsledkem používání azolových fungicidů v zemědělství dochází k selekci rezistentních kmenů, které většinou nesou mutace v genu cyp51A. Právě to následně komplikuje i léčbu pacientů s aspergilózou. Přestože incidence infekcí nereagujících na léčbu azolovými antimykotiky narůstá, nebyla prevalence azolové rezistence v České republice dosud studována. Prvním cílem této diplomové práce proto bylo zjistit prevalenci azolové rezistence u klinických kmenů A. fumigatus v ČR a studovat molekulárně-genetický podklad této rezistence. Druhým cílem bylo zjistit prevalenci rezistence a její genetické mechanismy v půdním prostředí a zkoumat vztahy mezi půdními a klinickými rezistentními izoláty. Pro tyto účely bylo ze tří zdravotnických zařízení shromážděno v letech 2019-2021 celkem 301 klinických izolátů A. fumigatus. Dále bylo získáno 300 půdních... Aspergillus fumigatus is a widespread saprophytic fungus. As an opportunistic pathogen it can also cause aspergillosis, which can manifest itself as a life-threatening infection in immunocompromised patients. Azole derivatives constitute first-line therapy of aspergillosis, but the increase worldwide of azole-resistant A. fumigatus strains represents a serious clinical problem. So far most of the molecular mechanisms causing azole resistance are dependent on the cyp51A gene. Studies of the last decade have shown that primarily because of the use of azole fungicides in agriculture, azole-resistant strains of A. fumigatus are selected in the environment. This subsequently complicates the treatment of patients with aspergillosis, and the incidence of infections unresponsive to treatment with azole antifungals increases. Increasing incidence of azole antifungals-resistant infections has become a growing concern: yet the prevalence of azole resistance has not yet been assessed in Czech Republic. Therefore, the first objective of this thesis was to determine Czech Republic's prevalence of azole resistance in clinical strains of A. fumigatus.
