[cze][eng] Tato bakalářská práce nese název Problematika smrti a umírání z pohledu zdravotnického záchranáře. Téma smrti i umírání vždy bylo součástí lidského života, a to především života ve zdravotnictví. Pohledy na smrt a její definici se po staletí měnily, stejně jako se měnila a vyvíjela péče o umírajícího. Smrt je všudypřítomná a od zdravotnického záchranáře se očekává, že je schopen se s ní vyrovnat, akceptovat ji, potažmo o ní mluvit. Současně musí být na takto těžkých výjezdech profesionálem, který zachovává humánnost i ohleduplnost v přístupu k pacientovi či pozůstalým. Hlavním cílem práce bylo zjistit, jak zdravotničtí záchranáři vnímají problematiku smrti a umírání v podmínkách přednemocniční neodkladné péče (PNP), a dále pak porovnat zkušenosti zdravotnických záchranářů se smrtí a umíráním v přednemocniční neodkladné péči. Prostřednictvím těchto cílů jsme následně stanovili výzkumné otázky: Jaké jsou zkušenosti zdravotnických záchranářů se smrtí a umíráním v podmínkách přednemocniční neodkladné péče? Jak vnímají zdravotničtí záchranáři problematiku smrti a umírání v přednemocniční neodkladné péči? Práce je členěna na část teoretickou a část praktickou. V teoretické části se zabýváme definicemi smrti a umírání, jejich historií i etickou rovinou. Dále pak popisujeme smrt i umírání přímo v přednemocniční neodkladné péči, jejich specifika, a v neposlední řádě pozici zdravotnického záchranáře. V rozsáhlejší, empirické části práce jsme použili kvalitativní výzkumné šetření za využití polostrukturovaného rozhovoru se zdravotnickými záchranáři pracujícími na vybraném středisku zdravotnické záchranné služby. Získané odpovědi byly analyzovány pomocí metody otevřeného kódování a kategorizace. Interpretaci dat jsme provedli technikou "vyložení karet". Z výzkumného šetření vyplynuly odpovědi na obě výzkumné otázky. Porovnali jsme zkušenosti zdravotnických záchranářů s danou problematikou a rozebrali jejich pohled na umírání i smrt v PNP. Z výsledků práce vyplynulo, že zdravotničtí záchranáři považují v přednemocniční péči za daleko problematičtější oblast umírání než samotné smrti. Dále se drtivá většina záchranářů shodla, že jsou pro ně psychicky nejnáročnějšími výjezdy k dětskému pacientovi, zejména pak k resuscitaci dítěte. Dále jsme z analýzy zjistili, že se zdravotničtí záchranáři po skončení studia necítili dostatečně informováni a vzděláni v oblasti smrti i paliativní medicíny. Práce tak může posloužit jako doplňkový edukační materiál pro studenty končících ročníků zdravotnický záchranář. Výsledky této práce mohou také sloužit jako podklad pro hlubší studium či výzkumná šetření. This bachelor thesis is titled The Issues of Death and Dying from the Perspective of a Paramedic. The topic of death and dying has always been a part of human life, especially in health care. The views on death and its definition have been constantly changing throughout the centuries, as there was a change in the care for the dying. Death is omnipresent and the paramedic is expected to cope with it, accept it, or else talk about it. At the same time the paramedic has to act professionally during difficult responses in the field, he or she is expected to act humane and with respect towards the patient or the grieving. The main goal of the thesis was to determine how the paramedics perceive the issue of death and dying in the conditions of pre-hospital medical care (PMC), and also to compare their experience in these conditions. Through this goal were then set down questions for this study: What are the experiences of paramedics with death and dying in the conditions of pre-hospital medical care? How do they perceive the issue of death and dying in these conditions? The thesis is divided into the theoretical and practical part. In the theoretical part is concerned with definitions of death and dying, their history and ethical level. Death and dying are then described when happening directly in the of pre-hospital medical care, their specifics, and also the position of paramedics themselves. In the more extensive, empirical part of the thesis there was employed a qualitative inquiry with the use of semi-structured interview with paramedics working at a previously chosen centre of emergency medical services. Acquired responses were analyzed by using the method of open coding and categorization. The data were interpreted by a technique of card sorting. The inquiry provided answers for both of the research questions. We compared the experiences of paramedics with the given issues and interpreted their perception of death in PMC. The results express that paramedics consider the course of dying in pre-hospital medical care more problematic than the death itself. Also, the majority of paramedics agree that the most mentally challenging are the medical responses involving children, especially their resuscitation. The results of the analysis also showed that paramedics after finishing their studies did not feel informed and educated enough in the field of death and palliative medicine. The thesis thus may serve as a supplementary educational material for students of final years of the paramedic study programme. The results may also serve as a foundation for a more profound studies or inquiries.
