Imunitní monitorace a imunoterapie po alogenní transplantaci krvetvorných kmenových buněk ABSTRAKT Alogenní transplantace krvetvorných buněk (aloHSCT) je vysoce specializovaná terapeutická metoda, která je potenciálně kurativní pro řadu hematologických i nehematologických onemocnění. Navzdory kontinuálním pokrokům v transplantační medicíně je prognóza pacientů stále limitována řadou potransplantačních komplikací, z nichž dominují relaps původní malignity a reakce štěpu proti hostiteli (Graft-versus-host disease, GvHD). Ty ovlivňují nejenom přežití pacientů, ale také snižují jejich kvalitu života, proto hledáme nové léčebné modality, které by jejich prognózu zlepšily. Jedním z nadějných moderních postupů v léčbě GVHD je podání buněk s imunomodulačním potenciálem (imunoterapie). V řadě evropských center, včetně našeho, jsou pacientům po aloHSCT v indikaci steroid-refrakterní GvHD (SR-GvHD) podávány mesenchymální kmenové buňky (MSC). Novou buněčnou populací se slibým potenciálem potlačit GvHD a zároveň potencovat reakci proti leukémii (Graft-versus-leukemia GvL) jsou invariantní NKT lymfocyty. Zavedení aplikace těchto buněčných populací do klinické praxe musí předcházet řada preklinických studií a i v průběhu klinického podávání musí probíhat kontinuální optimalizace výrobních a aplikačních postupů. Immune monitoring and adoptive immunotherapy following allogeneic hematopoietic stem cell transplantation - ABSTRACT Allogeneic hematopoietic stem cell transplantation (aloHSCT) is a specialized therapeutic method that is potentially curative for a number of hematological and non-hematological diseases. Despite continuous advances in transplant medicine, the prognosis of patients is still limited by a number of post-transplant complications, dominated by relapse and graft-versus- host disease (GvHD). These complications affect the survival of patients, but also reduce their quality of life. So, we are looking for new treatment modalities that would improve their prognosis. One of the promising procedures in the treatment of GVHD is the administration of cells with immunomodulatory potential (immunotherapy). In a number of European centers, including ours, mesenchymal stem cells (MSCs) are administered to patients after alloHSCT in the steroid-refractory GvHD (SR-GvHD) indication. Invariant NKT lymphocytes are a new cell population with a promising potential to suppress GvHD and at the same time they potentiate the graft-versus-leukemia (GvL) effect.
