Až 20% pacientů s IBD (inflammatory bowel diseases, zánětlivá střevní onemocnění) je diagnostikováno v dětském věku, průběh onemocnění bývá často závažnější a léčba obtížnější než u dospělých pacientů. Proto je nutno ji zahájit a případně změnit včas, aby se předešlo rozvoji komplikací. Časné rozpoznání relapsu hraje významnou roli pro další průběh onemocnění a celkovou prognózu pacienta. Změny v laboratorních parametrech (např. hodnota fekálního kalprotektinu) mohou významně předcházet rozvoji klinických příznaků a mohou být indikací k provedení kontrolní endoskopie nebo přímo k eskalaci či změně terapie. V této práci jsme se zaměřili zejména na pacienty po chirurgické resekci střeva, kteří tvoří jednu z rizikových skupin. Udává se, že až 70 % pacientů s Crohnovou nemocí dospěje během prvních 10 let od diagnózy k operačnímu výkonu na střevě. Dle výsledků se zdá být nadějným prediktorem relapsu onemocnění sérový albumin, který může souviset s celkovým nutričním stavem organismu. Ověřili jsme také efektivitu využití fekálního kalprotektinu a CRP jako prediktorů relapsu. Testování kalprotektinu přímo ve střevním resekátu se jako využitelný prediktor neukázalo, vyšší počet kalprotektin-pozitivních buněk na zorné pole nebyl asociován s endoskopickým relapsem onemocnění. Up to 20 % of patients with IBD (inflammatory bowel disease) is diagnosed in childhood, the course of the disease tends to often be more severe and the treatment more difficult than in case of adults. Therefore, it is necessary to commence or eventually change the treatment in order to prevent development of complications. Early detection of relapse plays significant role for further course of the disease and general prognosis of the patient. Changes to laboratory parameters (e.g. to the level of fecal calprotectin) can significantly prevent the development of clinical symptoms and may indicate the necessity for control endoscopy, further escalation or change in therapy. The primary focus of this thesis are patients who underwent surgical bowel resection, who constitute one of the risk groups. Studies show that as many as 70 % of patients suffering from Crohn's disease reach the point of bowel surgery within the first 10 years of the diagnosis. According to the results, a promising predictor of relapse of the disease seems to be serum albumin, which may relate to the overall nutritional status of the organism.
