Nádory hlavy a krku (HNSCC) jsou vysoce heterogenní skupinou nádorů. Rizikovými faktory jsou zejména kouření a konzumace alkoholu. Část těchto nádorů je spojena s infekcí lidským papilomavirem (HPV), která významným pozitivním prognostickým faktorem. Tyto nádory se od nádorů negativních na HPV liší klinickopatologickými charakteristikami, nádorovým mikroprostředím i odpovědí na léčbu. Hypoxie je jevem běžně se vyskytujícím v nádorech včetně HNSCC a je považována za významný prognostický a prediktivní faktor. Objasnění míry hypoxie v souvislosti s infekcí HPV by mohlo částečně vysvětlit rozdíly v prognóze těchto pacientů a také umožnit vhodnější volbu terapie. Významným negativním prognostickým faktorem řady nádorů je také aspartát-β-hydroxyláza, jejíž role v HNSCC ale zatím zkoumána nebyla. Ve své práci jsem se zaměřila na detekci exprese hypoxických markerů a aspartát-β- hydroxylázy ve vzorcích od pacientů s HNSCC na úrovni mRNA pomocí kvantitativní PCR a na úrovni proteinu metodou multispektrální imunohistochemie. Míru exprese těchto markerů jsem porovnávala u nádorů rozdělených dle virové etiologie i jiných charakteristik jako je kouření, lokalizace nebo stádium nádoru. Nejvyšší vliv na expresi těchto markerů měla infekce HPV, ale na základě dosavadních výsledků nelze jednoznačně ukázat na vyšší míru... Head and neck cancers (HNSCC) are heterogeneous group of tumours. Risk factors are mainly smoking and alcohol consumption. Some of these tumours are associated with human papillomavirus (HPV) infection, which is a significant positive prognostic factor. These tumours differ from HPV-negative tumours in clinicopathological characteristics, tumour microenvironment and response to treatment. Hypoxia is commonly found in tumors including HNSCC and is a significant prognostic and predictive factor. Elucidating the degree of hypoxia in relation to HPV infection could partly explain the differences in prognosis of these patients and allow more appropriate choice of therapy. Aspartate-β-hydroxylase is also a significant negative prognostic factor in a number of tumours, but its role in HNSCC has not yet been investigated. I focused on the detection of hypoxic markers and aspartate-β-hydroxylase expression in samples from HNSCC patients at the mRNA level by quantitative PCR and at the protein level by multispectral immunohistochemistry. The expression levels of these markers were compared in tumors stratified according to viral etiology and other characteristics such as smoking, localization or tumor stage.
