Diagnostika Alzheimerovy nemoci je v současné době založena na stanovení přítomnosti specifických proteinů v cerebrospinální tekutině (CSF), případně jejich zobrazení pomocí pozitronové emisní tomografie. Obě tyto metody jsou invazivní a mohou pacienta vystavit jistému riziku a diskomfortu. Krevní biomarkery proto představují naději pro včasnou diagnostiku a monitoraci jedinců ve zvýšeném riziku rozvoje Alzheimerovy nemoci. Cílem diplomové práce bylo stanovit biomarkery v krvi pomocí ultrasenzitivní Simoa TM (Single molecular assay) technologie a ověřit jejich vztah k hodnotám v CSF. Druhým cílem bylo vysledovat vztah mezi krevními biomarkery a kognitivními markery Alzheimerovy nemoci. Prokázali jsme, že mezi sérovými, plazmovými a CSF koncentracemi p-tau 181 existuje pozitivní vztah. Negativní korelace byla sledována mezi koncentracemi p-tau 181 v krvi a výsledky MMSE testu, který je považován za standardní nástroj kognitivního hodnocení. Tato zjištění naznačují, že plazmatický a sérový p-tau 181 může výrazně pomoci ke klinické identifikaci neurodegenerativních onemocnění v primární péči. Negativní korelace plazmatického a sérového p-tau 181 s výsledky MMSE testu naznačuje, že by mohl být krevní p-tau 181 používán v populačních studiích k detekci jedinců s vysokým rizikem rozvoje Alzheimerovy... The diagnosis of Alzheimer's disease is based on the determination of specific proteins in cerebrospinal fluid (CSF) or their imaging using positron emission tomography. Both methods are invasive and can expose the patient to risk and discomfort. Blood biomarkers, therefore, represent hope for early diagnosis and monitoring of individuals at increased risk of developing Alzheimer's disease. The diploma thesis aimed to determine biomarkers in blood using the ultrasensitive Simoa TM (Single molecular assay) technology and to verify their relationship to the values in CSF. The second aim was to track the relationship between blood biomarkers and cognitive markers of Alzheimer's disease. We demonstrated a positive relationship between serum, plasma, and CSF p-tau 181 concentrations. A negative correlation was observed between the concentrations of p-tau 181 in blood and the results of the MMSE test, which is considered a standard cognitive assessment tool. These findings suggest that plasma and serum p-tau 181 may greatly help the clinical identification of neurodegenerative diseases in primary care.
