[cze][eng] Tato disertační práce je souborem 5 studií zaměřených na meningiomy baze lební. Anatomická studie popisuje dislokace zevní a vnitřních arachnoidálních membrán meningiomy baze lební. Získané poznatky je možné aplikovat během chirurgické resekce k predikci přítomnosti či absence arachnoidální disekční vrstvy a adherence kritických neuro-vaskulárních struktur. V české multicentrické studii meningiomů baze lební jsme zaznamenali celkové přežití pacientů 98,1 %, morbiditu 13,2 % a mortalitu 1,3 %. Zdokumentovali jsme prediktivní faktory histologického stupně, rozsahu resekce, klinických výsledků, rizika recidivy a progrese. Nejrizikovější lokalizací byly petroklivální meningiomy s origem v oblasti interpedunkulární a prepontinní cisterny. V prospektivní studii zaměřené na vyšetření čichu u olfaktoriálních meningiomů jsme zdokumentovali pravděpodobné prediktory zachování čichu (intaktní čich, menší objem, unilaterální operační přístup). Vyšetření čichu je klíčové pro volbu chirurgické strategie a hodnocení funkčních výsledků. V retrospektivní studii Simpson grade IV resekcí byla adjuvantní stereotaktická radiochirurgie asociována s delším přežitím bez progrese, dobou do progrese a celkovým přežitím. Vzhledem k vysokému procentu progresí (50 %) a dlouhému celkovému přežití pacientů (10 let 79 %),...This dissertation is a set of 5 studies focused on skull base meningiomas. The anatomical study explored the dislocation of the outer and inner arachnoid membranes by the skull base meningiomas. The findings can be applied during surgical resection to predict the presence or absence of the arachnoid dissection plane and the adherence of critical neurovascular structures. The Czech multicentric study documented overall survival 98.1 %, morbidity 13.2 %, and mortality 1.3 %. Predictive factors of histological grade, the extent of resection, clinical outcomes, risk of recurrence and progression were analyzed. Petroclival meningiomas originating within the interpeduncular and prepontine cisterns were the most surgically challenging. Our prospective study focused on the examination of olfaction in olfactory groove meningiomas. The predictors of olfaction preservation were intact olfaction, lower volume and unilateral surgical approach. Olfaction examination is essential to choose surgical strategy and evaluate functional outcome.
