[cze][eng] Tato bakalářská práce je zaměřena na regulační poplatky ve zdravotnictví, které byly zavedeny 1.1 2008 v České republice. Problematika regulačních poplatků ve zdravotnictví je v současné době velmi citlivým a diskutovaným tématem, které i po téměř roce stále plní přední stránky novin, časopisů a televizních pořadů. Práce je rozdělena na část teoretickou a část praktickou. V teoretické části uvádíme zásady, kdy se regulační poplatek hradí a kdy ne. Zmiňujeme se také o ochranném limitu a o administrativní práci, které jsou spojené s vybíráním regulačních poplatků. Pro představu, jak fungují regulační poplatky v sousedních státech, zde uvádíme situaci v zahraničí. V praktické části této práce jsme si zvolili tři cíle. Prvním cílem bylo zjistit, jak sestry vnímají zavedení regulačních poplatků ve zdravotnictví. Druhým cílem bylo zjistit, zda mají regulační poplatky vliv na poskytovanou ošetřovatelskou péči a třetím cílem našeho výzkumu bylo zjistit, co sestry považují za nejvíc problémovou oblast v rámci regulačních poplatků. Tyto cíle byly splněny. Na základě cílů byly stanoveny výzkumné otázky. Jak sestry vnímají regulační poplatky ve zdravotnictví, co by chtěly změnit ve vybírání regulačních poplatků, jaká by měla být poskytovaná ošetřovatelská péče z pohledu sester a co vidí jako nejvíce problémovou oblast v rámci regulačních poplatků. Výzkumné otázky byly zodpovězeny. Ke zpracování bakalářské práce bylo použito kvalitativního šetření, metody dotazování. Sběr dat byl realizován technikou nestandardizovaného, strukturovaného rozhovoru. Náš průzkum probíhal v Nemocnici České Budějovice a.s. a Nemocnici Strakonice a.s. Odpovědi sester byly zpracovány do dvaceti kazuistik. Šetření probíhalo v únoru 2009. Výsledky naší bakalářské práce by mohly sloužit jako zpětná vazba pro nemocnice, kde bylo šetření realizováno. Následně by mohly přispět k vytvoření jednotné formy pro vybírání regulačních poplatků a to jak z pohledu platby, tak i administrativní práce. Dále by výsledky mohly posloužit k podpoře spolupráce mezi pacientem a zdravotním personálem. Doporučila bych také školící akce pro zdravotnický personál, aby uměl zvládat problematické situace. Například jak přistupovat k pacientovi, který odmítá zaplatit regulační poplatek. Pravidelné školící akce bych dále také doporučila pro staniční a vrchní sestry v oblasti legislativy v rámci regulačních poplatků. This bachelor work is focused on regulation fees in public health care that sere introduced in the Czech Republic on 1/1/2008. The issue of regulation fees in public health care is a very delicate and discussed topic at present, still filling front pages of newspapers, magazines and television programs after almost a year. The work is divided into theoretical and practical part. The theoretical part lists the principles when the regulation fee is and is not to be paid. We mention also the protective limit and the office job related to collection of regulation fees. We describe the situation abroad to give an idea how regulation fees work in neighbouring states. The practical part of this work had three goals. The first goal was to find out how nurses perceive the introduction of regulation fees in public health care. The second goal was to find out whether the regulation fees affect the nursing care provided, and the third goal of our research was to find out what nurses consider the most problem-loaded area within the regulation fees. Those goals were met. Research questions were set on the base of the goals. How nurses perceive regulation fees in public health care, what they would like to change in the collection of regulation fees, what the provided nursing care should look like from the nurses{\crq} point of view and what they consider the most problem-loaded area within the regulation fees. The research questions were answered. Qualitative investigation, questioning method were used to process the bachelor work. The data collection was implemented in the technique of non-standardized structured interview. Our inquiry took place in the Nemocnice České Budějovice a.s. and the Nemocnice Strakonice a.s. hospitals. The nurses{\crq} answers were processed into twenty pieces of casuistic. The investigation took place in February 2009. The results of our bachelor work could serve as feedback for the hospitals where the investigation was implemented. Subsequently they could contribute to create a united form for collection of regulation fees, both from the point of view of payment and of office work. Further the results could serve to support cooperation between the patient and the health care staff. I would also suggest training events for health care staff so that they are able to master problem-loaded situations. For example how to approach a patient who refuses to pay the regulation fee. I would suggest regular training events also for ward sisters and head nurses in the area of legislation of regulation fees.
