Dizertační práce pojednává o strategii operační léčby zlomenin horního konce pažní kosti nově vyvinutými úhlově stabilními hřeby. Práce má dvě části - experimentální a klinickou. V experimentální části byly postupným zatížením testovány tříúlomkové zlomeniny standardně vytvořené z kadaverozních preparátů a osteosyntézované hřebem Targon PH. V klinické části byly zjištěny dlouhodobé klinické výsledky léčby zlomenin proximálního humeru hřebem Targon PH na vlastním souboru. V experimentální části práce byly potvrzeny výborné biomechanické vlastnosti těchto implantátů a jejich vhodnost pro ošetření tříúlomkových zlomenin. Dlouhodobé výsledky zjištěné v klinické části pak potvrdily efektivitu léčby všech typů zlomenin pomocí nitrodřeňového hřebu Targon PH. U dvouúlomkových zlomenin bylo zjištěno vysoké procento zhojení zlomenin, minimální množství komplikací a rovnoměrnost dosažených výsledků ve všech věkových skupinách, u třía čtyřúlomkových zlomenin pak signifikantní závislost funkčního výsledku a rozvoje osteonekrózy hlavice na kvalitě repozice. In experimental part were by the novel technique from cadavers prepared three part fractures and stabilized by the intramedulary nail Targon PH tested. In the clinical part were analysed long-term results of the treatment by this technique in own cohort of patients. Experimental part of the thesis showed excellent biomechanical features of the osteosynthesis and confirmed suitability of this technique for the treatment of humeral head fractures. Longterm results obtained in the clinical part then confirmed effectivness of the treatment by intramedullary nail Targon PH for all types of the fractures of the proximal humerus. In the group of the fractures of the surgical neck were found out majority of excellent or good results in all age groups with low rate of complications, in the group of the fractures of humeral head significant dependance of the clinical result and head necrosis development on the quality of the reduction of the fracture.
