[cze][eng] Psoriáza je chronické zánětlivé kožní onemocnění, jehož prevalence se v Evropě pohybuje mezi 1,5-4,7 %. Podle posledních poznatků se psoriáza manifestuje nejen typickými kožními projevy, ale také mnoha dalšími nemocemi, které výrazně zhoršují kvalitu života psoriatiků. Mezi tyto choroby patří zánětlivé gastrointestinální choroby: Crohnova nemoc, ulcerózní kolitida a celiakie. V zahraničí byla provedena řada studií, které dokazují, že prevalence zánětlivých střevních nemocí je u psoriatiků výrazně vyšší než v běžné populaci. Podobné studie nebyly v ČR dosud provedeny, proto jsme iniciovali tuto studii. Na základě poznatků ze zahraničních studií předpokládáme přítomnost specifických markerů celiakie, Crohnovy nemoci a ulcerózní kolitidy u pacientů s psoriázou. Pro studii ověřující tuto asociaci jsme zvolili design případů a kontrol. Do skupiny případů jsme zařazovali pacienty s chronicky stacionární formou psoriázy a do kontrolní skupiny pacienty s jinými dermatózami. Do studie nebyli zařazeni pacienti a kontroly trpící autoimunitními a chronickými onemocněními z důvodu minimalizace confounding. Výběr kontrolní skupiny respektoval distribuci věku a pohlaví pacientů zařazených do skupiny případů. Kontroly byly vybírány pomocí frekvenčního párování v poměru 1:2. U všech subjektů bylo provedeno...Psoriasis is a chronic inflammatory skin disease, the prevalence of which is around 1.5 % to 4.7% in Europe. According to recent findings, psoriasis is associated with multiple other diseases, which aggravate the patient's future quality of life. Gastrointestinal inflammatory diseases, such as Crohn's disease, ulcerative colitis and coeliac disease are included in psoriasis comorbidities. Many foreign studies have already demonstrated that the prevalence of gastrointestinal inflammatory diseases is significantly higher in psoriatic patients than in the common population. No similar study has been performed yet in the Czech Republic. We therefore initiated a project aiming at identifying the presence of specific markers and early stages of autoimmune gastrointestinal in patients with psoriasis. The study was designed as a hospital- based case-control study. Patients with chronic plaque psoriasis were enrolled as cases. The control group, selected based on a ratio of 1:2, consists of patients with other skin diseases. The cases and controls were subject to the same selection criteria: all persons suffering from chronic and autoimmune diseases were excluded.
