Úvod: Mozkový edém je dobře známá a fatální komplikace akutního jaterního selhání (ALF - acute liver failure). Vliv léčby arteficiálními eliminačními metodami na cerebrální poškození u ALF je většinou, pokud vůbec, monitorován měřením intrakraniálního tlaku. Cílem této práce je posoudit vliv metody frakční plasmatické separace a adsorpce( FPSA - Fractionated Plasma Separation and Adsorption) - jedné z nebiologických eliminačních metod, na intrakraniální tlak. Druhým cílem této práce je vyhodnotit možnou roli intracerebrální mikrodialýzy v monitoraci efektivity léčby akutního jaterního selhání metodou FPSA. Metody. Ve dvou experimentech na velkém zvířeti - praseti, byly použity dva různé chirurgické modely ALF. V prvním experimentu byla experimentální zvířat rozdělena na dvě skupiny, skupinu ALF, u které bylo navozeno pouze jaterní selhání a skupinu FPSA, kde bylo navozeno jaterní selhání a následně u ní proběhl jeden cyklus léčby metodou FPSA. U obou skupin byl měřen intrakraniální tlak a hodnoty byly následně statisticky porovnány. V druhém experimentu byla porovnávána data z monitorace mikrodialýzou a intrakraniální tlak mezi třemi skupinami pokusných zvířat, tedy mezi skupinou s pouhým navozením ALF (ALF skupina), skupinou s ALF, u které proběhl šestihodinový cyklus léčby metodou FPSA (FPSA skupina), a... Background: Cerebral edema is a well-known and fatal complication of acute liver failure (ALF). The influence of the treatment by artificial liver devices on the cerebral damage caused by ALF is usually monitored by only measuring the intracranial pressure (ICP). The aim of this work was to determine the influence of Fractionated Plasma Separation and Adsorption (FPSA), nonbiological artificial liver device, on the intracranial pressure. The second goal of this work was to determine the potential role of cerebral microdialysis in monitoring of the efficiency of fractionated plasma separation and adsorption (FPSA) treatment of ALF. Methods: Two types of surgical resection model of ALF were used in two separate experiments in pigs. In the first study data from monitoring of the intracranial pressure of ALF group (animals with ALF only) and FPSA group (animals with ALF treated by FPSA) were compared. In the second study data from monitoring by cerebral microdialysis and ICP of the ALF group (animals with ALF only), FPSA group (animals with ALF treated by FPSA) and SHAM group (animals with only laparotomy) were compared using statistical analyses.
