Sepse a její komplikace jsou v současné době nejčastější příčinou úmrtí v důsledku multiorgánového selhání na jednotkách intenzívní péče. Cílem práce bylo posoudit nejvhodnější rutinně použitelný marker s dostatečnou diagnostickou senzitivitou a specificitou pro časný záchyt rozvoje sepse.Pro práci byl použit vybraný soubor dat pacientů z oddělení s intenzívní péčí a pacientů z oddělení neonatologie Nemocnice České Budějovice a.s..U pacientů byla provedena stanovení koncentrace parametrů: PCT, CRP a IL-6.Pro diagnózu a hodnocení změny klinického stavu ve vztahu k sepsi je výhodný PCT. Pro monitorování stabilního průběhu a klinického stavu je užitečné a postačující sledování dynamiky CRP. U novorozenců klesá senzitivita PCT u sepse asi díky nezralosti jater a imunitního systému, proto se jeví podle zkušeností získaných v Nemocnici České Budějovice a.s. vhodnější místo PCT stanovovat IL-6. Sepsis and its complications are currently the most frequent cause of death owing to multi organ failure at intensive care units. The aim of this thesis was to evaluate the most suitable usable marker with sufficient diagnostic sensitivity and specificity for early catch of sepsis development. For my thesis a chosen data collection patients of intensive care units and patients of neonatology department in Nemocnice a.s. (Hospital Inc.) in České Budějovice was used. In patients, determinations of following concentration parameters were carried out: PCT, CRP, IL-6. For diagnosis and evaluation of clinical state change in relation to sepsis PCT is convenient. For monitoring of steady course and clinical state it is useful and sufficient to watch dynamics of CRP. In newborns the sensitivity PCT decreases in sepsis probably due to immaturity of liver and immunity system. That is why according to experience gained in Nemocnice a.s. (Hospital Inc.) České Budějovice it is more convenient to determine IL-6 instead of PCT.
