Úvod: Prokalcitonin (PCT) je protein s řetězcem o 116 aminokyselinách s molekulovou hmotností 13 kD. Je tvořen parafolikulárními buňkami štítné žlázy. Zánětový prokalcitonin je tvořen jen 114 aminokyselinami. Prokalcitonin, který detekujeme v plazmě při probíhajícím zánětu, není tvořen C-buňkami štítné žlázy, ale v játrech, v menší míře pak v plicích, ledvinách, varlatech, prostatě a tenkém střevě. Cíle: 1. Porovnat hladiny a dynamiku C-reaktivního proteinu (CRP) a prokalcitoninu (PCT) při zánětlivém procesu u tří vybraných pacientů s rozdílnými diagnózami. 2. Porovnat počty vyšetřených PCT dle indikujících oddělení VFN v letech 2009, 2012, 2015. 3. Sledovat dynamiku počtu vyšetřených PCT v letech 2001-2016. Metody: Hladiny CRP a PCT byly stanoveny v laboratoři ÚLBLD VFN Praha v období 20. 5. -17. 6. 2016. CRP bylo měřeno imunoturbidimetrickou metodou na analyzátoru Unicel DxC 880i. PCT bylo měřeno elektrochemiluminiscenční metodou na analyzátoru Cobas e411. Pro vyhodnocení byly zpracovány grafy s hladinami obou analytů u 3 pacientů v kritickém stavu. Dále byla zpracována data potřebná pro vyhodnocení cílů č. 2 a 3. uvedených výše. Výsledky: Byla prokázána větší senzitivita i specificita PCT na rozdíl od CRP při odlišení infekčního a neinfekčního systémového zánětu. 1)U pacienta D. P. klesala... Introduction: Procalcitonin (PCT) is a 13 kD protein with a chain of 116 amino acids. It consists of parafollicularcells of the thyroid gland. Inflammatory procalcitonin is made up of 114 amino acids. Procalcitonin from plasma in the course of inflammatory process, does not consistof C-cells of the thyroid gland but it is generatedin the liver, and, to a lesser extent, in the lungs, kidneys, testes, prostate and small intestine. Objectives: 1. To compare the levels and the dynamics of C-reactive protein (CRP) and procalcitonin in the inflammatory process. in three patients with different diagnoses. To compare the number of examined PCT according to indicated VFN (General University Hospital) departments in the years 2009, 2012 and 2015. 3. To study the dynamics of the number of examined PCT in the years 2001- 2016. Methods: The levels of CRP and PCT were measured in the ÚLBLD laboratory of the General University Hospital in Prague in the period from May 20to June 17, 2016. The CRP was measured by immunoturbidimetricanalyzerUnicelDxC 880i. The PCT was measured by theCobas analyser using the electrochemiluminescence technology.
