Dědičné senzomotorické neuropatie (HMSN), taktéž známé pod názvem Charcot-Marie- Tooth disease (CMT) jsou klinicky i geneticky heterogenní skupina nejčastějších dědičných nemocí ovlivňujících periferní nervstvo. Tyto nemoci se obecně vyskytují v prevalenci 5 - 40 nemocných na 100 000 obyvatel. CMT byla poprvé popsána v roce 1886. Vzhledem k velkému množství různých druhů mutací je klasifikace HMSN nejednotná. Základní dělení je dle vedení vzruchu nervem na demyelinizační (CMT 1) a axonální (CMT 2) typ. Dále se pak klasifikuje převážně dle typu dědičnosti a fenotypového projevu. Autozomálně dominantní CMT 1 rozdělujeme na 4 základní formy - CMT 1A až D. Také CMT 2 můžeme rozlišit na jednotlivé genetické podtypy jako u CMT 1, nebo lze přistoupit ke klasifikaci na základě fenotypových projevů. Vedle autozomálně děděných HMSN jsou dalšími typy dědičných senzomotorických nemocí pohlavně vázané CMT, intermediární CMT, Déjerine- Sottas syndrom, kongenitální hypomyelizační forma a hereditární neuropatie s tendencí k tlakovým parézám. Jednotlivé typy HMSN jsou způsobeny mutacemi v různých genech nacházejících se na různých chromosomech. Nejčastějším typem CMT je autozomálně dominantní typ CMT 1A, kdy dochází k duplikaci v genu PMP22. Přestože tato onemocnění nejsou primárně smrtelná, velice ovlivňují... Hereditary motor and sensory neuropathy (in short HMSN or hereditary sensomotoric neuropathy) also known as Charcot-Marie-Tooth disease (CMT) is a clinically and genetically heterogeneous group of diseases which are the most frequent disorders affecting peripheral nervous system. The prevalence of these illnesses is generally 5 - 40 people per 100 000 inhabitants. CMT was first described in the 1886. Because of a large number of various types of mutations classification of HMSN is disunited. The main division of CMT depends on the median motor conduction velocity (and also on the part of nerve that was damaged). It is demyelinating (CMT 1) type and axonal (CMT 2) type. Further classification depends on the mode of heredity and phenotypic expression. Autosomal dominant CMT are divided into four main types - CMT 1A to D. Similar, CMT 2 could be distinguished by genetical subtype as well or, the classification can follow phenotypic expression. Beside the autosomal inherited HMSN, other types of hereditary sensomotoric diseases do exist: intermediate CMT, X-linked CMT, Déjerine-Sottas syndrom, congenital hypomyelination neuropathy and hereditary neuropathy with liability to pressure palsy.
