Stejně jako u ostatních orgánů i zde se neustále zvyšuje množství pacientů v terminálních stadiích onemocnění na čekacích listinách, ale nedochází k adekvátnímu navýšení dárcovských orgánů. V naší experimentální práci jsme se zaměřili na rozvoj výzkumu řešení, které se z dlouhodobého hlediska zdá jako nejúspěšnější a tím je transplantace orgánů od dárce s nebijícím srdcem (NHBDs) v protokolu ex vivo transplantace plic (EVLP) na zvířecím modelu (potkan kmene Wistar). Jedná se o metodu světově klinicky již zavedenou (v ČR jen experimentálně), která je neustále předmětem dalšího výzkumu. Na podkladě dříve prováděných studií jsme se v první experimentální části práce změřili na možný protektivní vliv hyperkapnické ventilace na ischemicko-reperfúzní (IR) poškození plic při EVLP. Studie prokázala, že hyperkapnická ventilace má protektivní vliv na vznik volných kyslíkových radikálů (ROS) u IR poškození plic, ale pouze pokud je využita v období reperfúze. Ve druhé experimentální studii jsme navázali na velmi aktuální téma vlivu pohlaví na IR poškození plic při EVLP u dárce s nebijícím srdcem. As with other organs, the number of patients on waiting lists in terminal stages of their diseases is also constantly rising but there is not an adequate increase in donor organs. We focused our experimental work on the development of research concerning the solution which is most successful in a long-term perspective, i.e. organ transplants from non- heart-beating donors (NHBDs), in an ex vivo lung transplant protocol (EVLP) on an animal model (Wistar rats). This is a method which is clinically established worldwide (in the Czech Republic only experimentally) and is constantly subject to further research. Based on earlier studies, we focused the first experimental part of this work on the potential protective effect of hypercapnic ventilation on ischemia-reperfusion (IR) lung injury in EVLP. The study proved that the hypercapnic ventilation has a protective effect on the generation of reactive oxygen species (ROS) in IR lung injury, but only when used in the period of reperfusion.
