Souhrn Předpoklady a cíle: Vysoký výskyt mutací a cytogenetických abnormalit u pacientů s myelodysplastickým syndromem (MDS) naznačuje přítomnost defektů v mechanismech DNA reparace. První část práce se zaměřuje na monitoraci genové exprese DNA reparačních genů a její změny v průběhu progrese onemocnění. Ve druhé části studie bylo použito sekvenování nové generace k detekci jednonukleotidových polymorfismů (SNP) a mutací v DNA reparačních genech a byla hodnocena možná asociace těchto genetických abnormalit s rizikem rozvoje MDS. Metody: Expresní profilování 84 DNA reparačních genů bylo provedeno na CD34 pozitivních buňkách kostní dřeně pacientů s MDS. Skríningová kohorta sestávala z 28 pacientů a exprese vybraných genů byla dále hodnocena v rozšířené kohortě 122 pacientů se všemi podtypy MDS. Sériové vzorky byly použity k monitoraci exprese genů RAD51 a XRCC2 v průběhu progrese onemocnění. Imunohistochemický průkaz RAD51 rekombinázy byl proveden u trepanobioptických vzorků. Cílené sekvenování exonových oblastí 84 DNA reparačních genů bylo provedeno ve skríningové kohortě MDS pacientů. Real-Time PCR byla použita ke zhodnocení genotypu vybraných SNP v populační studii. Výsledky: Geny RAD51 a XRCC2 vykazovaly odlišnou expresi mezi MDS nízkého a vysokého rizika (p < 0,0001), zatímco exprese genu RPA3 byla... Background: The high incidence of mutations and cytogenetic abnormalities in patients with myelodysplastic syndrome (MDS) suggests the involvement of DNA repair mechanism defects in the pathogenesis of this disorder. The first part of this work was focused on monitoring of gene expression of DNA repair genes in MDS patients and on their alterations during disease progression. In the second part, next generation sequencing was used to detect single nucleotide polymorphisms (SNPs) and mutations in DNA repair genes and their possible association with MDS development was evaluated. Methods: Expression profiling of 84 DNA repair genes was performed on bone marrow CD34+ cells of patients with MDS. Screening cohort consisted of 28 patients and expression of selected genes was further validated on larger cohort of 122 patients with all subtypes of MDS. Paired samples were used for monitoring of RAD51 and XRCC2 gene expression during disease progression. Immunohistochemical staining for RAD51 recombinase protein was done on samples acquired by trephine-biopsy. Targeted enrichment resequencing of exonic parts of 84 DNA repair genes was performed on the screening cohort of MDS patients.
