Alergie na včelí a vosí jed ohrožuje pacienta závažnou, mnohdy život ohrožující anafylaktickou reakcí po bodnutí hmyzem. Správná diagnostika a léčba je tudíž pro pacienta s touto diagnózou zcela zásadní a patří do rukou erudovaného alergologa. Cílem naší práce bylo poskytnout komplexní pohled na tyto nemocné, a to z mnoha různých úhlů pohledů, z pohledu epidemiologického, diagnostického a terapeutického. Nejprve jsme se zaměřili na odhalení prediktorů těžké reakce po bodnutí hmyzem, dále jsme hodnotili přínos aktuálně dostupných laboratorních metod pro diagnostiku alergie na hmyzí jed a sledovali jsme i změny imunologických parametrů v průběhu léčby imunoterapií hmyzím jedem. Všechny tyto dílčí záměry se nám podařilo splnit. Jako prediktory těžké reakce jsme odhalili věk (riziko narůstá s věkem), bodnutí do oblasti hlavy a mužské pohlaví. Rovněž jsme zhodnotili přínos jednotlivých diagnostických metod a navrhli jsme i možný způsob, jakým diagnostiku alergie na hmyzí jed optimalizovat. V průběhu léčby imunoterapií hmyzím jedem jsme zaznamenali statisticky významné změny imunologických parametrů. Z důvodu malého počtu bodnutí hmyzem v průběhu léčby jsme však zatím nemohli ověřit klinickou relevanci těchto změn. Hymenoptera venom allergy threatens the patients with severe, often life threating anaphylactic sting reaction. Proper diagnostics and treatment are therefore crucial for the patient with this diagnosis and should be performed by an experienced allergist. We aimed to provide a comprehensive look at these patients, from the epidemiological, diagnostic and therapeutic point of view. Initially, we focused on predictors of severe systemic sting reactions. Then we followed by evaluation of the utility of available laboratory diagnostic methods and by monitoring of immunological parameters during venom immunotherapy. We met all these goals. We detected higher age, sting localization in the head region and male gender as predictors of severe systemic sting reaction. We also evaluated the clinical utility of all diagnostics methods, and we even suggested the possible way, how to optimize the diagnostic procedure in patients with Hymenoptera venom allergy. We noticed significant changes in immunological parameters during venom immunotherapy. Due to a low number of stings during treatment, we have not been able to evaluate the clinical relevance of these changes yet.
