Aortální stenóza (AS) a mitrální regurgitace (MR) jsou nejčastějšími a zároveň nejčastěji operovanými chlopenními vadami u dospělé populace. Léčba předcházející chirurgickému či katetrizačnímu řešení chlopenní vady je cílena především na ovlivnění symptomů, zatímco kauzální farmakologickou léčbu k dispozici nemáme. Předpokladem účinného zásahu do patogeneze nejčastějších chlopenních vad je detailní poznání mechanismů jejich vzniku. Kalcifikující onemocnění aortální chlopně, považované v současné době za aktivní proces s účastí celé řady buněk, signálních molekul a transkripčních faktorů, má mnoho společného s patogenezí aterosklerózy, přesto až 50% pacientů s degenerativní AS nevykazuje významnou aterosklerózu. V naší práci jsme ve vzorcích kalcifikované aortální chlopně měřili hladiny osteoprotegerinu (OPG), který prostřednictvím blokády interakce RANKL/RANK (receptor activator of nuclear factor κB ligand/RANK) zasahuje do procesu kalcifikace a buněčné diferenciace během progrese AS. Prokázali jsme významně vyšší tkáňové koncentrace OPG ve skupině pacientů s izolovanou AS oproti skupině s koincidencí AS a koronární aterosklerózy, čímž jsme podpořili hypotézu o odlišné patogenezi kalcifikujícího onemocnění aortální chlopně v obou klinických situacích. Také v případě MR je poznání patogeneze klíčem k... Aortic stenosis (AS) and mitral regurgitation (MR) are the most common valvular heart diseases in the adult population. Additionally, they are the most likely to need surgical intervention. Treatment preceding surgical or percutaneous correction of a valvular defect is targeted primarily on symptoms, since causal pharmacological treatment is lacking. A detailed understanding of the mechanisms behind valvular heart disease is necessary to effectively intervene in the pathogenesis. Calcified aortic valve stenosis, currently considered an active process consisting of a wide range of cells, signaling molecules, and transcription factors, is largely consistent with the pathogenesis of atherosclerosis; however, about 50% of patients with calcified AS do not exhibit significant atherosclerosis. Our work was focused on measuring osteoprotegerin (OPG) levels in the tissue of calcified aortic valves. OPG acts as decoy receptor for RANKL and affects calcification and differentiation of various cells during progression of AS. We found significantly higher tissue levels of OPG in patients with isolated AS compared to patients with concurrent AS and coronary atherosclerosis.
