[cze][eng] Predikce terapeutické odpovědi při léčbě afektivních poruch repetitivní transkraniální magnetickou stimulací MUDr. Jakub Albrecht Abstrakt Úvod: Repetitivní transkraniální magnetická stimulace (rTMS) je efektivní a bezpečná neuromodulační léčba mnoha neuropsychiatrických onemocnění. Depresivní porucha rezistentní na léčbu (TRD) se stává celosvětově hlavní příčinou morbidity a mortality. Design studie byl naturalistický observační. Metody: Soubor (2016-2018) obsahuje 39 depresivních rezistentních pacientů (STAR*D ≥ 3). Parametry TMS byly: 10 dní, 10 Hz, energie 100 % motorického prahu (MT), 1500 pulzů v 15 trainech nad levou dorzolaterální prefrontální kůrou. Škály pro subjektivní hodnocení byly podány před a po poslední stimulaci: Zungova sebeposuzovací stupnice deprese (SDS), Škála vnímaného stresu (PSS), Beckův inventář úzkosti (BAI) a Zkrácený inventář depresivních příznaků (QIDS- SR). Medikace nebyla měněna. Výsledky: Subjektivní účinek byl statisticky významný se středním poklesem: u SDS 10 bodů (ze 75 ±8,16 na 65 ±9,55) a 59 % pacientů o ≥10 % oproti výchozímu stavu; v PSS o 4 body (29 ±5,34 na 25 ±5,90) a 62 % o ≥10 %; v BAI 4 body (46 ±13,72 na 42 ±11,51) a 54 % o ≥10 %; v QIDS-SR 6 bodů (17 ±3,91 na 11 ±5,05) a 72 % o ≥10 %. Treatment resistant depression (TRD) is becoming the leading cause of morbidity and mortality. The design was narutallistic and observational. Methods: The cohort (2016-2018) contains 39 depressed patients (STAR*D grade ≥3). The parameters of TMS were: 10 days of 10 Hz stimulation with an energy of 100 % of motor evoked potential (MEP), 1500 pulses in 15 trains over the left dorsolateral prefrontal cortex. Self-reporting scales were administered prior to and after the final stimulation: Zung's Self-Rating Depression Scale (SDS), Perceived Stress Scale (PSS), Beck's Anxiety Inventory (BAI) and Quick Inventory of Depressive Symptomatology (QIDS-SR). Co-medication was not altered.
