Tato bakalářská práce je zaměřena na problematiku aloimunizace matky v těhotenství a zabývá se její prevencí, diagnostikou a managementem. V České republice by se tato komplikace bez zahájení patřičné léčby vyskytla přibližně u 2000 žen ročně. Těhotná je ohrožena v případě, že je Rh(D) negativní a plod zdědí Rh(D) pozitivitu po otci. Rozvoji aloimunizace lze zabránit včasným preventivním podáním anti-D globulinu. I přes systematicky organizovaný systém profylaxe dochází dnes stále u malého počtu těhotných k imunizaci. Plody jsou ohroženy rozvojem anémie v děloze a po porodu potom rozvojem hyperbilirubinémie. U nejtěžších případů intrauterinní anemizace plodu je řešením intraumbilikální transfuze erytrocytů. Cílem práce bylo retrospektivně posoudit výsledky léčby pomocí intraumbilikální transfuze v Centru fetální medicíny. Kvantitativní část výzkumu je doplněna o 2 modelové kazuistiky hydropsu plodů. Kvantitativní část výzkumu analyzuje výsledky všech IUT provedených v od roku 1991 do roku 2021 v Centru fetální medicíny gynekologicko-porodnické kliniky VFN a 1. LF UK v Praze. Pomocí tabulek a grafů byly následně data analyzována a podrobně popsaná. Z výzkumné části této práce vyplynulo, že léčba plodů pomocí IUT je velmi úspěšná a zvyšuje šanci plodů na příznivou poporodní adaptaci. Z výsledků této... The bachelor thesis focuses on the issue of maternal alloimmunization in pregnancy and deals with diagnostic and management of the alloimmunization. This complication would occur in approximately 2000 women in the Czech Republic per a year without a treatment. A pregnant woman is at risk if she is Rh(D) negative and the fetus inherits Rh(D) positivity from father. The development of alloimmunization can be prevented by early preventive dosage of anti-D globulin. Despite a systematically organized system of prophylaxis, a small number of pregnant women still experience immunization. Fetuses are at risk of developing anemia in the uterus and than after birth by developing hyperbilirubinemia. In the most severe cases of intrauterine anemia, the solution is intraumbilical transfusion of erythrocytes. The aim of this study was to summarize the outcomes of IUT in retrospect. The quantitative practical part is completed with 2 model study cases of hydrops fetalis. The quantitative part of the research analyzes the results of all IUTs performed in the period from 1991 to 2021 in the Center of Fetal Medicine of the Gynecology and Obstetrics Clinic of the General Hospital and the First Medical Faculty of Charles University in Prague.
