Prognostické markery chirurgické léčby maligních plicních nádorů Úvod: Plicní karcinom je nejčastěji se vyskytující malignitou v dospělé populaci ve vyspělých zemích. Zvolení ideální strategie léčby plicního karcinomu je závislé na včasné diagnostice tohoto onemocnění, jelikož přežití a prognóza onemocnění jsou silně ovlivněny klinickým stadiem, ve kterém je léčba zahájena. Vedle konvenčních diagnostických metod lze diagnostiku doplnit a zpřesnit stanovením imunochemických markerů. Cíl: Žádný z biomarkerů zatím nebyl vyhodnocen jako dostatečně specifický pouze pro jeden typ nádoru. V naší práci jsme se pokusili sestavit panel několika markerů, které by svou specifičností mohly být užitečné jak ke zhodnocení prognózy, tak i k diagnostice nádorů, zejména v situacích, kdy nelze odebrat validní histologický vzorek. Dále jsme stanovovali souvislost mezi klinicko-patologickými parametry plicního karcinomu a hladinou imunochemických markerů a vliv hladiny imunochemických markerů na prognózu pacientů. Metodika: Do naší studie bylo zařazeno 109 pacientů s plicním karcinomem a 109 osob s negativní onkologickou anamnézou v kontrolní skupině. Pacienti s plicním karcinomem byli operováni na Chirurgické klinice FN a LF UK v Plzni mezi 17. dubnem 2018 a 26. srpnem 2020. U každého pacienta byla stanovena hladina CEA, NSE,... Prognostic markers of surgical treatment of malignant lung tumours Summary: Introduction: Lung cancer is the most common malignancy in adults in western countries. The ideal strategy of treatment depends on early diagnosis because the clinical stage strongly impacts the prognosis. Among conventional diagnostic methods, we can analyse immunochemical markers to make the diagnosis more precise. Aim: None of the biomarkers has been evaluated sufficiently specific for one type of tumour. In our work, we tried to find a panel of immunochemical markers, which could be useful as well for the assessment of prognosis as for diagnosis. Especially in situations when taking a biopsy is impossible. The next aim of our study was to evaluate the context between clinical-pathological factors and the level of immunochemical markers and their influence on prognosis. Methods: In our study, we classified 109 patients with lung carcinoma. In the control group were also 109 persons. All 109 patients with NSCLC underwent lung resection at the Department of Surgery of University hospital in Pilsen between 17th April 2018 and 26th August 2020.
