Traumatická poranění mozku představují závažný zdravotní problém. Kromě celé řady dalších komplikací mohou být spojeny s poruchami endokrinních funkcí hypothalamo-hypofyzární jednotky. Cílem naší práce bylo zjistit, zda je chronický subdurální hematom, jako specifický typ traumatického poškození mozku spojen s rizikem rozvoje hypopituitarismu. V prospektivním sledování jsme vyšetřili soubor 49 pacientů krátce po chirurgickém ošetření subdurálního hematomu a následně v odstupu 3 a 12 měsíců a 10 pacientů retrospektivně 12 měsíců po chirurgickém zákroku. Pacienty jsme podrobili sérii vyšetření včetně dynamických testů pro posouzení všech hypofyzárních funkcí. Poruchu gonadotropní osy jsme zjistili u 9 (25,7 %) vyšetřených jedinců v akutní fázi a u 3 (12 %) odstupu 12 měsíců od operačního zákroku. Porucha sekrece růstového hormonu byla v akutní fázi přítomna u 23 (46,9 %) jedinců. V odstupu 12 měsíců byl deficitu růstového hormonu přítomen u 15 jedinců (46,9 %). Významné poruchy klinicky nejvýznamnějších os, kortikotropní a thyreotropní, jsme v našem souboru nezaznamenali. Předmětem druhé části práce jsou nové možnosti diagnostiky poruchy adrenokortikotropní osy. Cílem naší práce bylo zhodnotit a porovnat spolehlivost paralelně stanovených koncentrací kortizolu v séru a ve slinách při ACTH testu. Pomocí... Traumatic brain injury represents serious medical problem. Besides various types of complications, endocrine dysfunction of hypothalmo-pituitary units can occur. Aim of our work was to determine whether chronic subdural haematoma, as specific type of traumatic brain injury, is associated with a risk of developing hypopituitarism as well. In a prospective study, pituitary functions in a cohort of 49 patients after surgery for chronic subdural haematoma, were evaluated. Patients were tested for hormonal deficits including dynamic tests, soon after surgery, in 3 and 12 months thereafter. 10 patients were tested retrospectively 12 months after surgery. Gonadotropic axis was impaired in 9 patients (25,7 %) in acute phase and reamained impaired in 3 (12 %) after 12 months. Growth hormone deficiency was present in 23 (46,9 %) patients during the acute phase and in 15 (46,9 %) patients in evaluation 12 months after the surgery. Serious hormonal deficiencies due to impairment of the most important axes (HPA a thyrotropic) were not proved in our patients.
