[cze][eng] V současné době patří chronická myeloidní leukémie (CML) mezi onkologická onemocnění, která jsou nejlépe prostudovaná na molekulární úrovni. Její vznik těsně souvisí s translokací mezi chromozomy 9 a 22, která vede ke vzniku fúzního genu bcr-abl kódujícího fúzní protein Bcr-Abl. Genová modifikace způsobuje nežádoucí zvýšení aktivity tyrosin-kinázy (TKA) kódované genem abl. Po zavedení léčby imatinib mesylátem a dalšími inhibitory tyrosin-kinázy (ITK), které se označují jako ITK druhé generace, se výrazně zlepšila kvalita života i délka přežití pacientů trpících CML. Současná cílená terapie však pouze zastavuje průběh nemoci a snad jen výjimečně ji vyléčí. Jedním z hlavních důvodů je, že se nádorové buňky nechovají při léčbě pasivně. Poměrně často dochází k mutacím, které mají za následek selekci klonu, který je na užitou farmakoterapii rezistentní; či k amplifikaci fúzního genu bcr-abl. Moderní farmakoterapie založená na ITK též není s to zasáhnout nádorové kmenové buňky, které se stávají při přerušení léčby zdrojem relapsu nemoci. Jedinou současnou možností vyléčení CML je transplantace allogenních hematopoetických kmenových buněk. Slabinami tohoto terapeutického přístupu jsou malý počet vhodných dárců a také vysoká morbidita a mortalita u nemocných příjemců. V současné době je rozšířeno...At present, chronic myeloid leukemia (CML) is one of the best understood oncological disorders at the molecular level. Its development is closely related to the translocation between chromosomes 9 and 22 that leads to the formation of the bcr-abl fusion gene encoding Bcr-Abl fusion protein. This gene modification results in an undesirable increase of the activity of the tyrosine kinase (TKA) encoded by the abl gene. After the introduction of imatinib mesylate and other tyrosine kinase inhibitors (TKI), referred to as the second generation TKI, the quality of life and the survival of patients with CML has greatly improved. Current drug treatment stops the progression of the disease and induces a remission; however, it only rarely, if ever, results in a cure. One of the main reasons is that cancer cells do not behave passively during the treatment. Frequently, the mutations of the gene lead to the selection of a clone, that is pharmacotherapy-resistant; or to an amplification of the bcr- abl. fusion gene. Furthermore, modern drug therapy based on TKIs does not eradicate tumor stem cells, which are the source of the leukemia relapse.
