Souhrn Studium potenciálního významu pravastatinu v prevenci cholestatického poškození jater. Obstrukční cholestáza je klinický syndrom doprovázející četná onemocnění jater. Časná diagnóza a příslušná léčba obstrukčního ikteru je velmi důležitá, neboť neléčený stav vede k ireparabilním změnám v játrech. Vzniká tak jaterní fibróza, která později přechází do biliární cirhózy se všemi jejími důsledky (portální hypertenze, jícnové varixy, jaterní selhání). Kauzální léčba v podobě chirurgického odstranění obstrukce přitom není často možná. Proto je nezbytné hledání především farmakoterapeutických přístupů, které mohou rozvíjející se onemocnění příznivě ovlivňovat. V předkládané disertační práci byl studován vliv pravastatinu, inhibitoru HMG-CoA reduktázy, na jaterní poškození v průběhu chronické obstrukční cholestázy u potkanů. Pravastatin, patřící do skupiny široce používaných statinů, oplývá kromě účinků hypolipidemických také účinky protizánětlivými a antioxidačními, tzv. účinky pleiotropními. Při volbě pravastatinu pro tuto studii byl rozhodující jeho pozitivní efekt pozorovaný u několika nemocných s cholestázou a hydrofilní povaha molekuly nevyžadující metabolizmus v postižených játrech a umožňující kompenzační eliminaci ledvinami. Obstructive cholestasis is a clinical syndrome accompanying numerous liver diseases. Early diagnosis and appropriate treatment of obstructive jaundice is very important because untreated condition leads to irreparable changes in the liver. This gives rise to liver fibrosis, which later passes into biliary cirrhosis with all its consequences (portal hypertension, esophageal varices, liver failure). The causal therapy such as surgical removal of the obstruction is quite often impossible. Therefore it is necessary to search for pharmacotherapeutic approaches that can positively modulate the developing disease. In the present thesis, the effect of pravastatin on the liver damage during chronic obstructive cholestasis was investigated in rats. Pravastatin, belonging to a group of widely used inhibitors of HMG-CoA reductase, possesses not only lipid-lowering action but also anti-inflammatory and antioxidant effects, so called pleiotropic effects.
