Cíl studie: Arytmická bouře u pacientů se strukturálním srdečním onemocněním je život ohrožující situace, nepříznivě ovlivňující prognózu. Katetrizační ablace je možnou účinnou metodou v léčbě arytmických bouří. Paroxysmy polymorfních komorových tachykardií/fibrilace komor (pKT/FK) u nemocných s ischemickou chorobou srdeční mohou být spouštěné monomorfními komorovými extrasystolami. Tyto lze také odstranit katetrizační ablací. Cílem naší studie bylo analyzovat akutní i dlouhodobou účinnost této terapie. Metody: Do souboru jsem zařadily 50 pacientů ( průměrný věk 59±13 let, 8 žen); 38 s ischemickou chorobou srdeční, 5 s idiopatickou dilatační kardiomyopatií, 6 s arytmogenní dysplazií pravé komory a jeden nemocný měl kombinovanou etiologii srdečního onemocnění. Všichni podstoupili katetrizační ablaci z důvodu arytmické bouře. Průměrná ejekční frakce levé komory ( EFLK) byla 29±11 %. U 9 pacientů po infarktu myokardu (IM) vznikla arytmická bouře v důsledku fokálně spouštěné pKT/FK. Odstup od IM byl 3dny-171 měsíců. Arytmická bouře se objevila buď časně po IM (do 1 měsíce, skupina 1, n=5) nebo v delším odstupu (více než 1 měsíc, skupina 2, n=4). Výsledky: Arytmická bouře byla potlačena v 86% pacientů. Během následného sledování (18±16 měsíců) u 24 nemocných nedošlo k recidivě komorových tachykardií (48%).... Aims: Electrical storm (ES) adversely affects prognosis of patients with structural heart disease and may become a life-threatening event. Catheter ablation (CA) has been proposed for the treatment of ES. Frequent episodes of polymorphic ventricular tachycardias/ventricular fibrillation (VT/VF) in patients with coronary artery disease can be triggered by monomorphic ventricular premature beats (VPBs) and thus, amenable to catheter ablation. Our goal was to evaluate the efficacy of CA ablation both in acute and long-term suppression of ES. Methods: Fifty consecutive patiens (age 59±13 years, 8 female) with coronary artery disease (38), idiopathic dilated cardiomyopathy (5), arrhythmogenic right ventricular cardiomyopathy (6), and/or with combined aetiology (1) underwent CA for ES. Mean left ventricular ejection fraction (LVEF) was 29 ± 11%. All patients underwent electroanatomical mapping, and CA was performed to abolish all inducible ventricular arrhythmias. Catheter ablation of electrical storm due to focally triggered polymorphic VT/VF was performed in nine patients. All 9 patients had previous myocardial infarction (MI) (interval of 3 days to 171 months).
