Disertační práce má několik stanovených cílů. Za prvé odhadnout fyziologické hladiny parathormonu u novorozenců velmi nízké porodní hmotnosti. Za druhé zhodnotit stav zásobení vitaminem D u matek a jejich novorozenců s velmi nízkou porodní hmotností při narození a při propuštění z nemocnice. Za třetí porovnat parametry kostního metabolismu a sledované charakteristiky časné nemocnosti ve skupině novorozenců velmi nízké porodní hmotnosti s hladinami 25-hydroxy vitamin D [25(OH)D] v pupečníkové krvi ≤ 25 nmol/l a > 25 nmol/l. A za čtvrté posoudit, zda je deficit vitaminu D v pupečníkové krvi rizikovým faktorem pro rozvoj syndromu dechové tísně novorozence. Jedná se o soubor prospektivních observačních studií zahrnující nezralé novorozence s porodní hmotností pod 1500 g. Parametry minerálového a kostního metabolismu byly analyzovány z pupečníkové krve, séra a moče novorozenců v průběhu hospitalizace (parathormon, 25OHD, S-Ca, S-P, ALP, U-Ca, U-P) a ze séra těhotných žen těsně před porodem [25(OH)D]. Kostní mineralizace byla hodnocena pomocí kostní denzitometrie. Fyziologické rozmezí hladin PTH u předčasně narozených novorozenců bylo odhadnuto jako 1.6 - 9.3 pmol/l. Od druhého měsíce života byla prokázána statisticky významná korelace mezi PTH a 25(OH)D (Rho = -0.71, p = <0.0001). Hladina 25(OH)D byla <... Firstly, the aim of dissertation work was to estimate physiological parathyroid hormone (PTH) levels and their relationship with bone metabolism parameters in otherwise healthy preterm newborns with birth weight 1000-1500 g. Secondly, to evaluate vitamin D status in mothers and their very low birth weight infants (VLBW) at birth and at discharge with currently recommended supplementation of vitamin D. Thirdly, to compare clinical outcomes of VLBW infants with 25-hydroxy vitamin D [25(OH)D] levels ≤ and > 25 nmol/l in umbilical cord blood and finally to evaluate umbilical cord vitamin D as a risk factor for respiratory distress syndrome in preterm infants. It is a set of prospective observational studies involving immature newborns with birth weight below 1500 g. The parameters of mineral and bone metabolism were analysed in umbilical cord blood and newborn serum and urine during hospitalisation (PTH, 25-hydroxy vitamin D, S-Ca, S-P, ALP, U-Ca, U-P) and in pregnant women before delivery (25-hydroxy vitamin D).
