Souhrn Cílem prospektivní studie bylo u kompresí optického chiazmatu (OC) kvantifikovat přínos optické koherenční tomografie (OCT), resp. zrakových evokovaných potenciálů (VEP) pomocí měření tloušťky peripapilární vrstvy nervových vláken (RNFL - retinal nerve fibre layer) a perifoveální vrstvy gangliových buněk (GCL - ganglion cell layer), resp. měřením implicitního času a amplitudy při tzv. pattern-reversal VEP (R-VEP) a motion-onset VEP (M- VEP). Soubor a metodika: Do souboru bylo zařazeno 16 pacientů (32 očí) netrpících vedle útlaku OC žádným jiným závažným onemocnění očí či zrakové dráhy. Podmínkou pro zařazení do studie byla indikace dekompresivního operačního výkonu. Vyšetření zrakové ostrosti, zorného pole, RNFL, GCL, R-VEP a M-VEP se uskutečnilo jednou předoperačně a 3x pooperačně (1 týden, 3 a 6 měsíců). Na předoperačním snímcích MR (magnetická rezonance) mozku byl určen stupeň útlaku OC (tzv. grade) od 0 po 4. Pro část analýzy dat byl soubor rozdělen na skupinu s žádným nebo minimálním tlakem na OC (grade 0-1) a jednoznačným tlakem na OC (grade 2-4). Výsledky: Medián tloušťky peripapilární globální RNFL byl 87 µm, perifoveální nazální GCL 41,2 µm a temporální GCL 44,2 µm. U komprese OC jsme prokázali nižší hodnoty RNFL v nazálním (63,5 µm) i temporálním (65 µm) kvadrantu než je průměr pro danou... Use of optical coherence tomography and visual evoked potentials recording in preoperative and postoperative monitoring of patients with optic chiasm compression The objective of this prospective study is to explore the benefits of optical coherence tomography (OCT), resp. visual evoked potentials (VEP), in cases of optic chiasm (OC) compression by measuring the thickness of the retinal nerve fiber layer (RNFL) and the ganglion cell layer (GCL), resp. the implicit time and the amplitude resulting from pattern- reversal VEP (R-VEP) and motion-onset VEP (M-VEP). Material and methods: 16 patients (32 eyes) with chiasmal compression were included in the study. They presented no other pathology of the visual pathway or of the eye globe. The second inclusion criterion was a subsequent indication of decompressive surgery. Measurements of visual acuity, visual field, RNFL, GCL, R-VEP and M-VEP were performed once preoperatively and three times postoperatively (one week, 3 and 6 months postoperatively). The degree (grade 0-5) of chiasmal compression was determined on preoperative brain magnetic resonance imaging (MR).
