Východisko: Práce prezentuje široký pohled na problematiku zajištění dostatečné nutriční podpory (enterální výživy) s vysokým množstvím bílkoviny pro cílovou skupinu pacientů v intenzivní péči. Pacienti na těchto odděleních (ARO, JIP) mají rozdílné metabolické nároky v různých časových intervalech jejich akutního onemocnění. Pochopení podstaty metabolismu základních makronutrientů se všemi důsledky na organismus je základ pro porozumění metabolismu v intenzivní péči s významným vlivem na odlišné nutriční požadavky těchto pacientů a v naší maximalizace snahy přizpůsobit druh enterální výživy potřebám pacientů v intenzivní péči. Cíl: Cílem práce bylo zhodnotit vliv nové enterální formule Peptamen Intense na nutriční stav pacientů v intenzivní péči. Posoudit možnost dosažení nutričního cíle proteinů (energie) dle doporučení guidelines a zhodnotit další parametry, které ovlivňují management nutriční podpory u pacientů intenzivní péče. Metodika: Na pracovištích (ARO, JIP) v rámci celé ČR bylo do této práce vybráno deset pacientů podle jasně definovaných vstupních kritérií v období listopad 2017 až březen 2018. Následně byl u všech zařazených pacientů vyplňován Peptamen Intense Protokol (PIP) v průběhu jejich pobytu na intenzivní péči s podmínkou minimální enterální nutrice pět dní. Sledování bylo... Background: The paper presents a broad view of the issue of providing sufficient nutritional support (enteral nutrition) with a high level of protein for the target group of patients in intensive care. Patients in these types of departments (ARO, ICU) have different metabolic requirements at different time intervals of their acute illness. Understanding the essence of metabolism of essential macronutrients with all the consequences on the organism is the basis for understanding metabolism in intensive care with a significant influence on the different nutritional requirements of these patients and maximizing our efforts to adapt the type of enteral nutrition to the needs of intensive care patients. Objective: The aim of this work was to evaluate the effect of the new enteral formula Peptamen Intense on the nutritional status of patients in intensive care. Assess the possibility of achieving the nutritional target of proteins (energy) according to the guidelines. Evaluate other parameters that influence management of nutritional support in patients with intensive care.
