[cze][eng] Téma mé bakalářské práce zní "Problematika levorukých a leváků při fyzioterapii". V teoretické části práce se zabývám lateralitou, dominancí hemisfér a cévní mozkovou příhodou jako takovou. Dále na CMP nahlížím z pohledu fyzioterapie, kdy mě zajímají vývojová stádia iktu, rehabilitační léčba, postup při vyšetřování pacienta a fyzioterapeutické metody, které se dají v terapii využít. V teoretické části se ještě zabývám ergoterapií a kompenzačními pomůckami. Praktická část bakalářské práce je zpracovaná kvalitativní metodou. Pro vypracování vstupního a výstupního vyšetření jsem využila anamnestické údaje, rozhovor s pacientem, zdravotnickou dokumentaci a kineziologické vyšetření. Výzkum probíhal v Domově pro seniory Máj v Českých Budějovicích. Výzkumný vzorek tvořily dvě pacientky v subakutním stádiu CMP. Jedna pacientka byla levák a měla postiženou pravou dominantní hemisféru s levostrannou hemiparézou. Druhá pacientka byla pravák a měla postiženou levou dominantní hemisféru s pravostrannou hemiparézou. Fyzioterapie probíhala po dobu dvou měsíců (leden, únor 2016) 1× týdně. Ve dnech mé nepřítomnosti s pacientkami cvičily fyzioterapeutky zaměstnané v Domově pro seniory Máj České Budějovice. Účinnost terapie jsem posoudila na základě výsledků ze vstupního a výstupního vyšetření. Tato práce má za úkol zmapovat, zda je možné vytvořit stejné podmínky pro začleňování do běžného denního života pacienty po CMP. Jedná se o pacienta leváka s levostrannou hemiparézou, kdy je postižena pravá dominantní hemisféra a o pacienta praváka s pravostrannou hemiparézou, který má postiženou levou dominantní hemisféru. Dále hodnotím, který z pacientů se bude hojit rychleji a kvalitněji. Porovnání terapie pacientů má za cíl zjistit, zda jsou všechny léčebné techniky, ergoterapeutické pomůcky a fyzioterapeutické postupy uzpůsobeny situaci, kdy je postižena pravá nebo levá polovina těla. Výsledky výzkumné části bakalářské práce se shodují s tvrzením Josefa-Hermanna Zocheho (2006), který ve své knize uvádí, že se leváci po cévní mozkové příhodě zotavují rychleji a kvalitněji než praváci. Myslím si však, že toto tvrzení nelze považovat za zcela pravdivé a objektivní, protože se vyskytuje mnoho faktorů, které mohou zotavování pacientů po CMP výrazně ovlivnit. Z mých výsledků též vyplývá, že u obou pacientek došlo ke zlepšení zdravotního stavu. Také se mi potvrdilo tvrzení, že je velmi důležitá včasná léčba a rehabilitace, spolupráce veškerého ošetřujícího personálu, věk, motivace a spolupráce pacienta, podpora ze strany rodiny a přátel. Během terapie jsem nepostřehla žádnou rozdílnost v použitých léčebných technikách, ergoterapeutických pomůckách či fyzioterapeutických postupech ve spojitosti s lateralitou. Domnívám se, že není výrazný rozdíl v nabídce kompenzačních pomůcek pro pravoruké a levoruké pacienty. Pouze kompenzační pomůcky pro leváky jsou dražší a méně dostupné. The topic of my thesis is "Problems of left-handed in physiotherapy". The theoretical part deals with laterality, dominance and hemispheric stroke itself. Further I am describing hemispheric stroke from the point of view of physiotherapist, where I'm interested in the developmental stages of stroke rehabilitation therapy, the procedure for examining the patient and physiotherapy methods, which can be used in therapy. The theoretical part is dealing with occupational therapy and assistive devices. The practical part is a qualitative method. For the preparation of the input and output tests I used the history data, patient interview, medical records and kinesiology tests. The research was done at retirement home Máj in České Budějovice. The research sample consisted of two patients in the subacute stage CMP. One patient was left-handed and had affected the right hemisphere dominant left-sided hemiparesis with. The second patient was right-handed and had affected the left hemisphere dominant with right-sided hemiparesis. Physiotherapy lasted for two months (January, February 2016) 1× weekly. When I could not come, the patients had the therapy with physiotherapists, who normally work there. The effectiveness of therapy was based on the results of input and output tests. The main goal was to find out, if it is possible, to create eaqual conditions for inclusion in ordinary daily life of patients after stroke. There is a left-handed patient with left hemiparesis when there is affected right dominant hemisphere and right-handed patient with right-sided hemiparesis, which has affected his left dominant hemisphere. I evaluate which of the patients will heal faster and better. Comparing the therapy of each of them aims to determine whether all healing techniques, occupational therapy and physiotherapy equipment adapted to the situation where is affection of right or left half of the body. The results of the research part of the thesis agree with the statement Josef-Hermann Zoche (2006), who says in his book, that the lefties after a stroke recover faster and better than right-handed. But I think, that this statement can not be considered completely truthful and objective, because there are many factors that can influence retirement. From my resoluts it is also clear, that there was an improvement of both of the patients. It was also confirmed, that the early threatment and rehabilitation is very important. Also cooperation of medical stuff, age, motivation and cooperation of the patient, support from family and friends are necessary. During therapy, I have not noticed any difference in treatment techniques that I used, aids of occupational therapy or physiotherapy procedures in connection with laterality. I believe that there is no significant difference in supply assistive devices for right-handed and left-handed patients. Only aids for left-handers are more expensive and less available.
