Souhrn Úvod& cíle: Akutní interní onemocnění seniorů vede velmi často ke ztrátě svalové hmoty a soběstačnosti. Obnova svalové tkáně, zejména u geriatrických pacientů, je velice obtížná a tak může proběhlý akutní stav znamenat nutnost dopomoci druhé osoby formou následné sociální péče v domácím prostředí nebo v ústavech následné péče. Cílem naší studie bylo zjistit, zda aktivní přístup založený na časné rehabilitaci a nutriční podpoře mohou ovlivnit vývoj sarkopenie a soběstačnosti pacienta v průběhu akutního onemocnění. Metodika: Dvě stě pacientů, starších 78 let, přijatých k hospitalizaci s akutním onemocněním na geriatrické oddělení bylo zařazeno do prospektivní, randomizované, kontrolované studie. Pacienti byli randomizováni do kontrolní skupiny (standardní léčba, n=100) a intervenční skupiny (n=100). Intervence spočívala v podávání doplňků stravy (600 kcal, 20 g bílkovin denně) přidaných ke standardní dietě společně s intenzivní rehabilitací. Pacienti byli sledování po dobu 2 let od propuštění. Hodnocena byla tolerance nutričních doplňků a její vliv na spontánní příjem stravy během hospitalizace a skladba těla, svalová síla a soběstačnost během hospitalizace a v následných pravidelných kontrolách 2 roky od propuštění z nemocnice. Reversal of muscle tissue, especially in geriatric patients, is very difficult and thus can an acute illness leads to necessity a second person helping a form of subsequent social care at home or in institutions aftercare. The aim of our study was to determine whether an active approach based on early physiotherapy and nutritional supplementation legs to influence the development of sarcopenia and patient's self- sufficiency during the acute illness. Methods: Two hundred patients, older than 78 years, admitted to geriatric department with acute illness were enrolled in a prospective, randomized, controlled study. Patients were randomized to the control group (standard treatment, n = 100) and the intervention group (n = 100). The intervention consisted of the administration of nutritional supplements (600 kcal, 20 g protein daily) added to the standard diet and simultaneous intensive rehabilitation therapy.
