[cze][eng] Disertační práce: Využití robotické chirurgie v otorinolaryngologii Cílem této dizertační práce je přiblížit odborné veřejnosti využití robotické chirurgie v ORL oboru. V teoretické části práce je popsán robot a jeho použití. Tato část je rozdělena na historii vývoje robota, jeho vznik a použití v medicínském prostředí. Dále se tato část zabývá charakteristikami, bezpečností a principy práce s robotem, včetně popisu nejvíce používaného robotického typu v lékařství - systému da Vinci. Je vysvětleno využití robota v otorinolaryngologii a ostatních chirurgických oborech, včetně přípravy a pracovního postupu robotického operačního zákroku v ORL. Součástí je i definice indikačních kriterií pro transorální robotický přístup (TORS) a další rozšířené indikace použití robotické chirurgie v otorinolaryngologii, včetně transaxilární thyroidectomie. Ve speciální části je provedeno vyhodnocení dat ze souboru pacientů operovaných pro T1-T2 karcinomy kořene jazyka a supraglotis pomocí transorálního roboticky asistovaného zákroku TORS nebo klasického zevního přístupu a jejich srovnání v pooperačním období podle vybraných kriterií. The theoretical part deals with the description of robotic devices and their use. This part is divided into several chapters: the invention of robots, its development and its usage in medicine. The theoretical part also deals with specifications, safety and main principles of controlling. It also includes the description of the most commonly used robotic system in medicine - the daVinci system. Furthermore, the theoretical part explains, how robots are used in otorhinolaryngology and other surgery fields including the preparation and practical procedures in a robotic operation. The other part describes specific indication criteria for transoral robotic surgery (TORS) including extended uses in otorhinolaryngology, especially in transaxillary thyroidectomy. The special part contains the evaluation of resulting data from a group of otorhinolaryngology patients with T1 and T2 squamous cell carcinoma of base of tongue and supraglottis who were operated on robotically in comparison to traditional open surgery procedures.
