Porodnické intervence ve vztahu k poruchám pánevního dna - Abstrakt Jedním z hlavních cílů porodnických intervencí v druhé době porodní je prevence poranění pánevního dna a s ním spojenými poruchami. V současnosti nejčastěji užívané a zároveň i nejvíce diskutované intervence jsou chránění hráze a nástřih hráze (episiotomie). Většina poruch pánevního dna je subjektivního rázu a hledání vhodných nástrojů k objektivizaci a kvantifikaci závažnosti těchto poruch je klíčem k porovnání a nalezení nejefektivnějších intervencí. Cílem této disertační práce bylo nalézt a demonstrovat účinnost nejefektivnější modifikace chránění hráze. Dále porovnání dvou užívaných typů episiotomií stran poruch pánevního dna. Posledním cílem bylo nalézt konsensus ohledně nejvhodnějšího nástroje na hodnocení anální inkontinence. Ve studiích zabývajících se problematikou chránění hráze jsme popsali pomocí experimentu směr a míru deformace hráze při vaginálním porodu. Při následných počítačových simulacích na biomechanickém modelu hráze jsme prokázali, že Vídeňská modifikace chránění hráze, při které jsou prsty přiloženy na hráz 6 cm od zadní komisury laterálně na obě strany a 2 cm ventrálně a jsou v průběhu porodu k sobě přitlačeny o 1 cm bilaterálně, je schopná největšího snížení napětí na hrázi. Obstetric interventions and pelvic floor disorders - Abstract One of the principal objectives of obstetric interventions in the second stage of labor is prevention of pelvic floor trauma and associated pelvic floor disorders. The most commonly used and also most frequently discussed interventions are manual perineal protection and episiotomy. The majority of pelvic floor disorders are of subjective nature. Therefore, tools for objectification and severity quantification of these disorders are the key to reliable comparison and identification of the most effective interventions. The aim of the dissertation was to find the most effective modification of manual perineal protection, to compare two frequently used types of episiotomy regarding all pelvic floor disorders and finally to find a consensus on the most suitable instrument for anal incontinence severity assessment. Our studies concerning manual perineal protection experimentally described the direction and extent of perineal deformation during vaginal delivery.
