Úvod: Ischemicko- reperfuzní poškození představuje vzhledem k zásadnímu vlivu na prognózu pacientů s akutním infarktem myokardu (AIM) důležitý preventabilní faktor. Nejdůležitějším a nejlépe modifikovatelným parametrem ovlivňujícím rozsah tohoto poškození je čas do reperfuze. Snahám o zkrácení času do reperfuze u AIM, zejména s elevacemi ST segmentů (STEMI), bylo v posledních dvou dekádách věnováno mnoho úsilí, nicméně i přes významné pokroky v organizaci a kvalitě péče se velké části pacientů ani v současnosti nedaří ošetřit v rámci doporučených časových intervalů. Cíl: Cílem předkládané práce je potvrzení důležitosti vlivu času do reperfuze na preventabilní ischemické a reperfuzní poškození a prognózu pacientů v současné éře perkutánních koronárních intervencí (PCI). Dále si klade za cíl detailnější charakterizaci subpopulací pacientů s nejvyšším rizikem pozdní reperfuze a reperfuzního poškození a analýzu příčin k tomu vedoucích. Na základě získaných poznatků formulovat doporučení pro konkrétní preventivní opatření zaměřených na zkrácení času do reperfuze. Metodika: V uváděných původních pracích byla analyzována studijní populace z registru celkem 946 konsekutivních pacientů s STEMI anebo AIM s raménkovým blokem, kteří postoupili PCI v Kardiocentru Královské Vinohrady. Background: The ischemic-reperfusion injury represents an important preventable factor, due to its essential impact on prognosis of patients with acute myocardial infarction (AMI). One of the most important modifiers of the extent of the injury is the time to reperfusion. Great efforts to reduce delays in reperfusion have been made during the previous two decades, however, in spite of significant improvement in organization and quality of AMI treatment, especially ST- segment elevations (STEMI), many patients are not being treated within recommended time frames. Aim: The aim of this work was to acknowledge the importance of time to reperfusion in preventable ischemic and reperfusion injury and patient prognosis at the current era of percutaneous coronary interventions (PCI). Furthermore, we intended to characterize the patient subpopulations in the highest risk of delayed reperfusion and the analysis of its backgrounds. Based on the acquired data, specific recommendations for preventive measures aimed at reducing the time delay to reperfusion would be formulated.
