U pacientů po alogenní transplantaci hematopoetických kmenových buněk (HSCT) je lidský cytomegalovirus (CMV) jedním z nejzávažnějších virových patogenů. Jeho bližší určení by mohlo poskytnout informace o vlivu jednotlivých CMV genotypů na celkové přežití pacienta a některé závažné komplikace, jako je například reakce štěpu proti hostiteli (GvHD). Tato diplomová práce se zabývá retrospektivní genetickou analýzou 1877 vzorků od pacientů transplantovaných na Klinice dětské hematologie a onkologie FN Motol a Ústavu hematologie a krevní transfúze od roku 2002. DNA z biologických vzorků (především plné krve) byla izolována kitem Qiagen Blood DNA Mini, případně Qiagen DNA Mini a ve vzorcích byla prospektivně detekována přítomnost CMV DNA. Vzorky byly následně uchovány při -20 řC. Genotypizace byla provedena pomocí real-time PCR technologie zaměřené na geny 2 strukturních proteinů glykoproteinu B a glykoproteinu H za použití sekvenčně specifických primerů a sond. U 1343 vzorků (71,6 %) od 390 pacientů jsme prokázali infekci pouze jedním kmenem CMV, u 256 (13,6 %) vzorků od 113 pacientů jsme prokázali smíšenou infekci, způsobenou dvěma a více kmeny CMV. Nejčastěji prokázaný genotyp v "single" infekci byl u dětských i dospělých pacientů gB1/gH2 detekovaný u 118 (28,4 %) pacientů. Nejčastěji prokázanou... In patients after allogeneic haematopoietic stem cell transplantation (HSCT) is a human cytomegalovirus (CMV) one of the most important viral pathogens. Its detailed characteristic could provide information about the impact of each CMV genotype on overall survival of the patient, and some serious complications, such as graft versus host disease (GvHD). This thesis deals with retrospective genetic analysis of samples from 1877 patients transplanted at the Clinic of Pediatric Hematology and Oncology, University Hospital Motol and the Institute of Hematology and Blood Transfusion since 2002. DNA from biological samples (especially whole blood) was isolated kit Qiagen DNA Blood Mini or Qiagen DNA Mini and samples were prospectively detected presence of CMV DNA. Samples were subsequently stored at -20 řC. Genotyping was performed using real-time PCR technologies to the genes of 2 structural proteins glycoprotein B, glycoprotein H and using sequence specific primers and probes. In 1343 samples (71.6%) from 390 patients there was only one strain of CMV; in 256 (13.6%) samples from 113 patients have detected mixed infection caused by two or more strains of CMV.
