Ledviny jsou životně důležitým orgánem zajišťujícím vnitřní rovnováhu tělesných tekutin a iontů, čištění organismu od metabolitů a v neposlední řadě plní důležitou endokrinní roli. Funkce ledvin jsou řízeny celou řadou vzájemně se ovlivňujících mechanismů, které jsou schopny fungovat zcela autonomně bez centrálního řízení. Tyto mechanismy jsou řízeny humorálními systémy jak systémovými, tak tvořenými přímo v ledvinách a jejich vzájemná rovnováha udržuje optimální funkce ledvin. Porušením této rovnováhy dochází k poruše jak funkční, tak k progresu poškození ledvinné tkáně a ledviny tak ztrácejí schopnost plnit svou úlohu. Funkční nedostatečnost a rozvoj onemocnění ledvin je stále významným problémem i přes pokroky v medicíně. To je zapříčiněno i celou řadou rizikových faktorů, kde vedle věku, genetické predispozice, nízké porodní váze, hraje roli i vzestup civilizačních chorob jako je diabetes, vysoký krevní tlak, různá autoimunitní onemocnění, ale i špatné návyky jako nezdravý životní styl, kouření atd. Tyto faktory mohou vést k akutním nebo chronickým onemocněním ledvin z různých, často nejasných příčin. Otázkou pak zůstává včasná diagnostika a optimální léčba, která by měla zejména chránit ledvinu před dalšími progresy poškození, tak aby nedošlo k terminálnímu poškození ledvin. Tato práce proto... The kidneys are an essential organ that maintains the homeostasis of body fluid and ions, it filters metabolites and plays important endocrine role. Renal function is controled by many mechanisms with a close interaction that can work independently without central control. These mechanisms are regulated by both systemic and intrarenal humoral systems and their physiological balance sustains the optimal kidney fuction. Any disturbance of this balance leads to the impairments of renal function and progression of renal injury and thus the kidney works inappropriately. The renal insufficiency and diseases remain significant problem despite modern medicine. This is results of several risk factors such as age, genetic predisposition and low birth weight, increase of civilization diseases - diabetes, hypertension, autoimmune disorders, but also bad habits such as unhealthy lifestyle, smoking etc. These factors can lead to the acute or chronic kidney diseases often without known cause. Than the question is an early diagnosis and optimal treatment to preserve kidney function and stop the progression of terminal renal damage.
