SOUHRN Zánětlivé a protrombotické faktory hrají v procesu aterogeneze zásadní úlohu. Dyslipidémie i diabetes mellitus jsou spojeny s aktivací endotelu a leukocytů, které podporují zánětlivou infiltraci subendoteliálního prostoru a zhoršují endoteliální dysfunkci, přispívají tak k rozvoji předčasné aterosklerózy. U diabetiků navíc nacházíme množství odchylek u krevních destiček a koagulačních faktorů, které se dalšími mechanismy podílejí na procesu aterogeneze a mohou též přispívat k vzniku akutních aterotrombotických příhod. Proto jsou studovány látky, které by tyto procesy mohly příznivě ovlivnit. V této práci jsme u pacientů s hypercholesterolémií a s diabetem mellitem 2. typu vyšetřovali leukocytární expresi cytoadhezivních molekul, u diabetiků dále expresi trombogenních molekul na leukocytech a také solubilní endoteliální a trombogenní markery. U těchto pacientů jsme se zabývali dopadem hypolipidemické a antidiabetické léčby na tyto markery. Pacienti s těžkou hypercholesterolémií byli vyšetřeni před začátkem léčby a po 10 týdnech léčby atorvastatinem. Diabetici byli vyšetřeni před začátkem léčby a po 5 měsících podávání rosiglitazonu. Obě skupiny byly porovnány se zdravými kontrolami. Leukocytární exprese cytoadhezivních a trombogenních molekul byla měřena průtokovou cytometrií a kvantitativní real-time... Inflammatory and prothrombotic factors play a crucial role in the atherogenesis. Dyslipidemia and diabetes mellitus as well are associated with the endothelial and leukocyte activation, which facilitate the inflammatory infiltration of the subendothelial space and deteriorate endothelial dysfunction, and thus contribute to the development of premature atherosclerosis. Furthermore, diabetes is associated with a number of platelets and coagulation factors abnormalities that participate in atherogenesis by other mechanisms and may be involved in the acute atherothrombotic events progression. Therefore, the factors that could favourably influence these actions become the subject of interest. In this work, we examined leukocyte expression of cell adhesion molecules in patients with hypercholesterolemia and type 2 diabetes, and thrombogenic molecules on leukocytes in type 2 diabetes, and also soluble endothelial and thrombogenic markers. In these patient groups, we investigated the effect of lipid lowering and antidiabetic treatment on these markers. Patients with severe hypercholesterolemia were examined at the baseline and after 10 weeks of atorvastatin treatment.
