Sledování ukazatelů oxidativního stresu v kondenzátu vydechovaného vzduchu u pacientů s těžkým refrakterním astmatem. Úvod: Průduškové astma je chronické zánětlivé onemocnění dýchacích cest s bronchiální hyperraktivitou, na jehož patogenezi se podílí mnoho buněčných populací a jejich působků. V patogenezi astmatu hraje důležitou roli oxidativní stres, který je důsledkem působením volných reaktivních radikálů kyslíku (ROS) a dusíku (NOS) vzniklých při zánětu dýchacích cest a expozicí environmentálním škodlivinám (ozón, kouř cigaret). Produkty oxidativního stresu vedou k poškození buněčných struktur na úrovni biomembrán, proteinů a nukleových kyselin. Tíže astmatu dle literárních zdrojů koreluje s intenzitou oxidativního stresu. Cíl práce: Cílem práce bylo prokázat rozdíly v koncentracích ukazatelů oxidativního stresu v kondenzátu vydechovaného vzduchu (KVV) mezi pacienty s těžkým refrakterním astmatem (TRA) a zdravou kontrolní skupinou. Dále jsme sledovali vztah eozinofílie periferní krve (EC), koncentrace vydechovaného oxidu dusnatého (FeNO) k intenzitě oxidativního stresu. Metodika: U 40 pacientů s TRA byl proveden odběr KVV (Jaeger EcoScreen), který byl následně zpracován metodou kapalinové/plynové chromatografie a hmotnostní spektrometrie. Background: Asthma is chronic inflammatory disease associated with bronchial hyperreactivity. Various cells plays and cellular products are involved in asthma pathogenesis. Oxidative stress plays also a significant role in mechanisms of asthma via action of reactive oxygen (ROS) and nitrogen (NOS) species. These free radicals develop from inflammatory airway reaction and from exogenous exposure of air-pollution (ozone, cigarette smoke). ROS and NOS and its products cause damage of bio- membranes, proteins and nucleic acid. Several studies showed correlation of asthma severity to intensity of oxidative stress. Aim of the study: The main goal of the study was to describe differences in concentrations of various markers of oxidative stress in exhaled breath condensate of severe refractory asthma (SRA) patients and healthy controls (HC). Secondary aim was to show correlation of oxidative stress intensity to severity of eosinophilic inflammation described by peripheral blood eosinophilia or exhaled nitric oxide concentration.
