Název práce: Nastavení optimálního režimu vyšetřování markerů sledovaných klinicky významných infekcí u dobrovolných dárců krve Autor práce: MUDr. Daniela Dušková Vedoucí práce: prof. MUDr. Vladimír Tesař, Dr.Sc., MBA, FASN Tato disertační práce si klade za cíl přispět k diskuzi o rozšíření rutinního screeningového vyšetřování dárců krve v zařízeních transfuzní služby v České republice zavedením přímého vyšetřování vybraných infekčních agens molekulárně biologickými metodami. V úvodu teoretické části je připomenut stručným pohledem do historie vývoj a přelomové události v transfuzní medicíně. Následuje část věnovaná problematice infekčních chorob souvisejících s transfuzí a obecným vyšetřovacím postupům používaným v transfuzní medicíně při výběru dárců krve. Závěr teoretické části je věnován stávajícím možnostem screeningových testů pro vyšetřování infekčních markerů u dárců krve, včetně přehledu postupů, které jsou používané v České republice, Evropské unii a dalších vybraných státech. V praktické části je popsána prováděná studie, tj. screeningové vyšetřování souboru dárců krve rutinní vyšetřovací metodou (CMIA) a molekulárně biologickými metodami (RT-Real Time PCR) na přítomnost markerů HCV, HBV a HIV. Jsou popsány principy obou metod a vlastní postup vyšetřování v rámci studie. Je uveden souhrn... Vladimír Tesař, M.D., Dr.Sc., MBA, FASN The aim of this project is to contribute to the discussion about introducing the methods of molecular biology into the routine blood donor testing in the transfusions departments in the Czech Republic. The theoretical part includes a brief history and some turning points in transfusion medicine. The next part within the theoretical section is dedicated to the problems of infectious diseases concerning transfusion and the general examination processes used during the selection of blood donors. The end of the theoretical part concentrates on existing possibilities of markers' examination of clinically important infections in blood donors, including the list of processes performed in the Czech Republic, the European Union and other countries. The practical part describes this study, ie. the routine screening test of blood donors using the CMIA method (a routine method) and using RT-Real Time PCR method (a molecular biology method) for detecting infectious markers (HCV, HBV, HIV).
