Úvod a cíl práce: Předčasný porod je porod mezi 24. a 37. gestačním týdnem. Zhruba u 3% všech těhotenství dojde k předčasnému odtoku plodové vody (preterm premature rupture of membranes, PPROM), který je zodpovědný zhruba za jednu třetinu předčasných porodů. Mezi významné rizikové faktory patří infekce. Rozhodující vliv na typ a intenzitu imunitní odpovědi na mikroorganismus přítomný v intraamniální dutině má vrozená imunita a zvláště její část odpovědná za identifikaci tzv. PAMP, s patogenem asociovaných molekulových vzorů. Vrozená imunita má k jejich rozpoznání celou paletu specifických receptorů označovaných jako receptory pro nebezpečné vzory, PRR (Pattern Recognition Receptors). Nejvýznamnější z nich jsou TLR receptory. Jsou to membránové struktury, které se ale vlivem různých faktorů mohou odštěpit od membrány a fungovat jako solubilní formy. Naše studie si dala za cíl změřit hladiny solubilních forem těchto molekul, a to sTLR2 a sTLR4 v plodové vodě pacientek s PPROM a zhodnotit jejich možný diagnostický význam. Metody: Do studie bylo zařazeno 44 těhotných žen s gestačním stářím mezi 24. a 36. týdnem gravidity, Z nich 23 pacientek mělo histologicky prokázanou chorioamnionitidu. Z této skupiny mělo 9 pacientek zvláště závažný zánět, funisitidu. Zbylých 21 pacientek bylo bez zánětu. Dále byly... Background: Premature birth is a birth between 24th and 37th gestational weeks. Risk of preterm birth is in an increase of perinatal morbidity and mortality. Approximately in 3% of all pregnancies premature rupture of membranes (preterm premature rupture of membranes, PPROM) occurs. PPROM is responsible roughly for one-third of premature births. Among the important risk factors of PPROM infection is considered. The innate immunity has the decisive influence on the type and intensity of immune response to the microorganism present in the cavity, and is in particularly responsible for identifying the PAMP, pathogen-associated molecular patterns by specific receptors (PRR). The most important PRR are TLR receptors. They are primarily membrane expressed, although due to influence of many variable factors they can be shed from the membrane and can act in a soluble form. The aim of study our study is to measure the levels of soluble forms of PRR, especially sTLR2 sTLR4 and in amniotic fluid of patients with PPROM and to evaluate their possible diagnostic significance.
