Úvod: Příčinou akutního koronárního syndromu je ruptura nestabilního plátu, patogeneza je komplexní a předpokládá souběh řady mechanismů, a to složení plátu a nebo nerovnováhu trombotické aktivity krve. Cíle: Hlavním cílem práce bylo posouzení lokálních plasmatických hladin tkáňového faktoru (TF), inhibitoru tkáňového faktoru ( TFPI), tkáňových metaloproteináz (MMP-2, 3 a 9) a inhibitoru tkáňových metaloproteináz-2 (TIMP-2), vysoce selektivního CRP (hsCRP) a solubilního ligandu CD40 (sCD40L) u pacientů s akutním koronárním syndromem, se stabilní anginou pectoris v systémové cirkulaci, v koronárním sinu a stanovit časový průběh hladin u pacientů s akutním koronárním syndromem. Metodika a výsledky: Předmětem klinického hodnocení byli pacienti s ischemickou chorobou srdeční (ICHS). U 24 pacientů se stabilní formou ICHS (SCAD) byly během diagnostické koronární angiografie provedeny odběry z koronárního sinu (CS), femorální žíly (VF) a kmene levé věnčité tepny (LMCA). U 29 pacientů s akutním koronárním syndromem (ACS) bylo toto stanovení rozšířeno o kontrolní odběr žilní krve za 24 hodin a za 7 dní od prvního měření. Ve vzorcích byly stanoveny hladiny TF, TFPI, MMP-2, 3 a 9, TIMP-2, hsCRP a sCD40L. Plasmatické hladiny tkáňového faktoru byly signifikantně vyšší u pacientů s akutním koronárním syndromem než u... Pathogenesis is complex and requires various mechanisms and depends both on the composition of plaque and thrombotic activity of blood. Objectives: The main goal of this work was to assess local levels of plasma tissue factor (TF), tissue factor inhibitor (TFPI), tissue metalloproteinases (MMP-2, 3 and 9) and inhibitor of metalloproteinases-2 (TIMP-2), a highly selective C-reactive protein (CRP) and soluble CD40 ligand (sCD40L) in patients with acute coronary syndrome and with stable angina pectoris in the systemic circulation, the coronary sinus and to determine the time course of blood levels in patients with acute coronary syndrome. Methods and results: Study population include patients with coronary artery disease (CAD). In 24 patients with stable CAD (SCAD) at time of diagnostic coronary angiography, we withdrew samples from the coronary sinus (CS), femoral vein (VF) and left coronary artery (LMCA).
