Souhrn Nové přístupy v léčbě makulárních edémů sítnice. Intravitreální aplikace triamcinolon acetonidu. Hodnotíme účinnost intravitreální injekce triamcinolon acetonidu (IVTA) v dávce 4 mg u pacientů s makulárním edémem (ME) různé etiologie. V prospektivní studii je sledováno celkem 54 očí. 1. skupinu tvoří 30 očí s diabetickým makulárnám edémem (DME), 2. skupinu tvoří 16 očí s ME při oklusi sítnicové žíly a 3. skupinu představuje 8 očí s s cystoidním makulárním edémem (CME) po operaci katarakty. Všechny pacienty jsme vyšetřili před aplikací IVTA a při kontrolách za 1, 3, 6, 9 a 12 měsíců následovně: nejlépe korigovanou zrakovou ostrost (NKZO) na ETDRS tabulích, nitrooční tlak (NT) aplanační tonometrií, biomikroskopické vyšetření předního i zadního segmentu oka se zaměřením na makulu na štěrbinové lampě, dále byla provedena barevná fundusfotografie na digitální funduskameře a změřena makulární tloušťka (MT) optickou koherenční tomografií (OCT). Ve všech skupinách ME jsme prokázali redukci MT i zlepšení NKZO v průběhu i na konci sledovacího období. V 1. skupině (DME) se NKZO zlepšila z 0,19 na 0,23 a MT poklesla z 518µm na 375µm. Statisticky významné zlepšení NKZO (p<0,05) bylo prokázáno za 1, 3, 6 a 9 měsíců po aplikaci IVTA. Ve 12. měsíci bylo zlepšení nevýznamné (p=0,057). Redukce ME byla významná ve... We evaluate the efficacy of 4 mg intravitreal triamcinolone injection (IVTA) in patients with macular edema (ME) of various etiology. This prospective study included 54 eyes. Group I consisted of 30 eyes with diabetic macular edema (DME), group II included 16 eyes with ME after retinal vein occlusion and group III contained 8 eyes with cystoid macular edema (CME) after standard cataract surgery. All patients were examined prior to injection and at the 1, 3, 6, 9 and 12 months as followed: the best corrected visual acuity (BCVA) using ETDRS charts, intraocular pressure (IOP) by applanation tonometry, anterior segment and macula biomicroscopy at the slit lamp. Colour fundusphotography was done and macular thickness (MT) was measured by optical coherence tomography (OCT). In all groups of ME, the reduction of MT and improvement of BCVA was observed during and at the end of the study. Mean BCVA in group I improved from 0.19 to 0.23 and MT decreased from 518µm to 375µm. Statistically significant improvement of BCVA (p<0,05) was demonstrated for 1, 3, 6 and 9 months after IVTA. At the 12th month follow up improvement of BCVA was not significant (p=0,057).
