SOUHRN ÚVOD: Malou podskupinu nemalobuněčných karcinomů plic (NSCLC) představují tumory, jejichž karcinogeneze je navozena fúzí genu ALK s některým z dalších partnerů (nejčastěji EML4). Pacienti s přestavbou genu ALK, kteří jsou léčeni ALK inhibitory, mají výrazně delší dobu přežití. Vzhledem k tomu, že přestavba genu ALK je popisována dle současně platné WHO klasifikace u 4 - 5 % NSCLC, je nezbytně nutné najít optimální způsob identifikace pacientů vhodných pro tuto cílenou léčbu v rutinní diagnostické praxi. CÍL: V retrospektivní části studie bylo cílem a) nalezení imunohistochemického (IHC) postupu detekce proteinu ALK , který bude mít dostatečnou senzitivitu a specificitu, aby jej bylo možné použít jako screeningové metody při vybírání případů NSCLC pro následné testování přestavby genu ALK metodou fluorescenční in situ hybridizace (FISH) a b) zjištění diagnostické výtěžnosti "malých" biopsií, tj. endobronchiálních, transbronchiálních a transtorakálních biopsií a cytobloků, pro vyšetření přestavby genu ALK. V prospektivní části studie bylo cílem a) ověření zvolené IHC metodiky detekce exprese ALK proteinu v rutinním vyšetřování pacientů s NSCLC a b) korelace ALK statusu s morfologickým vzhledem tumorů a s vybranými klinickými údaji. METODIKA: V retrospektivní části studie bylo metodami IHC (tři... (Identification of optimal algorithm for effective diagnostics of non-small cell lung carcinoma with ALK gene rearrangement - implementation of the method and practical experience with routine diagnostics) INTRODUCTION: Smaller subgroup of non-small cell lung carcinoma (NSCLC) is represented by tumours with carcinogenesis iniciated by fusion of ALK gene with another partner (usually EML4). Patients with ALK gene rearrangement treated with ALK inhibitors have significantly prolonged survival. Since ALK gene rearrangement is described according to the current WHO classification in 4-5% of NSCLC, it is necessary to find the optimal way of identifying patients eligible for this targeted therapy in routine diagnostic practice. AIM: In the retrospective part of the study a) to find an immunohistochemical (IHC) detection procedure of ALK protein with specificity and sensitivity high enough to use this antibody as screening method for selecting NSCLC cases for fluorescence in situ hybridization (FISH) testing of ALK gene rearrangement and b) to determine diagnostic yield of "small" biopsies, i.e. endobronchial, transbronchial and transthoracic biopsies and cytoblocks, for ALK gene rearrangement testing.
