[cze][eng] Význam stanovení aktivovaného parciálního tromboplastinového času a porovnání citlivosti parciálních tromboplastinů k lupus antikoagulans, deficitu koagulačních faktorů a heparinu Dagmar Drábková Abstrakt Aktivovaný parciální tromboplastinový čas (aPTT) je koagulační test pro odhalování poruch hemostázy a nejpoužívanější test pro monitorování léčby nefrakcionovaným heparinem. V parciálním tromboplastinu jsou přítomny kontaktní aktivátory a fosfolipidy, které mohou významně ovlivnit citlivost reagencie ke koagulačním faktorům, lupus antikoagulans a heparinu. Cílem práce bylo porovnat citlivost parciálních tromboplastinů (s odlišným původem fosfolipidů a různými aktivátory) k lupus antikoagulans, ke sníženým a zvýšeným hladinám koagulačního plazmatického faktoru VIII a citlivost ke standardnímu heparinu. Dále na základě naměřených dat zjistit, zda některá z vybraných reagencií umožňuje odhalit současně deficit koagulačního faktoru, přítomnost inhibitoru a zároveň je vhodný k monitorování heparinu. Stanovení aPTT bylo provedeno s pěti reagenciemi s různým původem fosfolipidů a s rozdílnými aktivátory. Měření byla provedena na plně automatickém koagulometru BCSXP. Z naměřených dat jsme zjistili, že žádný z testovaných tromboplastinů neměl 100% citlivost ke všem zmíněným faktorům. In parcial tromboplastin are present activators of contact phase and phospholipids which can significantly affect the sensitivity of reagents to coagulation factors, lupus anticoagulans and heparin. The objective of this study was firstly to compare the sensitivity of parcial tromboplastins with different origins of fosfolipids and various activators to lupus anticoagulans, to decreased and increased levels of coagulation plasmatic factor VIII and sensitivity to standard heparin. Secondly, according to measured data to determine whether any of the selected reagents is suitable for monitoring heparin while being able to detect deficit of coagulation factors and presence of inhibitor.
