[cze][eng] SOUHRN VÝSLEDKŮ Prokázali jsme, že je možné vytvořit dobře fungující experimentální model, který věrně napodobuje klinickou praxi, umožňuje rozsáhlou chirurgickou instrumentaci, extenzivní sledování a monitoraci na různých patofyziologických úrovních a dovoluje tak nejen lépe pochopit jednotlivé patofyziologické děje, ale zároveň detailní analýzu jednotlivých terapeutických intervencí. Věříme, e takto etablovaný model, může i v budoucnu napomáhat k poznání patogeneze kritických stavů a dovolí např. testování nových diagnostických či léčebných přístupů. Na tomto modelu jsme dále přispěli do mozaiky patofyziologie akutního poškození ledvin v sepsi. Za podmínek adekvátně tekutinově resuscitované sepse a septického šoku se nejeví sní ení průtoku renální tepnou s následnou globální ischémií ledviny hlavním patogenetickým mechanizmem AKI. Zajímavým zjištěním bylo navýšení renálního ilního tlaku. Tato renální ilní kongesce zasluhuje bezpochyby nadále pozornost v klinice i experimentu. Jednou ze známých interakcí v MODS je kardio-renální syndrom, který se mů e rozvinout nejen u septických pacientů, ale i např. u pacientů s chronickým srdečním selháváním a jak napovídají některé poslední studie, právě u takových pacientů renální ilní kongesce mů e významně ovlivňovat nejen vlastní renální funkce, ale i v konečném...Sepsis and septic shock remain major cause of mortality in non-coronary intenisve care units. Prognosis of septic patiens worsens further in case of concomitant acute kidney injury. Pathophysiological pathways leading to renal dysfunction in sepsis remain unclear despite of enormous experimental and clinical research. Similarly, the role of extracorporeal blood purification techniques as an adjunctive treatment in sepsis is highly controversial. The aim of our study was to dynamically assess renal haemodynamic, microvascular and metabolic responses in a porcine clinically relevant model of septic shock. The same experimental model was used in experiments elucidating potential benefit effects of two distinct haemopurification methods on different biological responses to infectious insult.
