[cze][eng] Souhrn Chirurgické řešení primární hyperparathyreózy je vysoce účinnou metodou. Cílem je odstranění hyperfunkční tkáně příštítných tělísek a tím definitivně vyléčit primární hyperparathyreózu s dosažením normální hladiny kalcia. Přitom dochází ke zlepšení zdravotního stavu a vymizení nebo alespoň snížení obtíží. Komplikace nejsou časté a mortalita je minimální. Chirurgické řešení je definitivní, bezpečné a účinné. Na základě 101 chirugicko-anatomických pitev provedených na zemřelých byla vypracována standardní chirurgicko-anatomická preparace příštítných tělísek. Její základem byla preparace zkřížení a. thytroidea inf. a n. laryngeus recurrens. Úspěšnost identifikace tkáně parathyroidey byla 75%, nejčastější příčinou neúspěchu byla záměna za tkáň thymu, za lymfatickou uzlinu, za uzel štítné žlázy. Přitom byly testovány jednotlivé miniinvazivní přístupy. V další části práce je objasňován syndrom kostní, metabolický a biochemický a předložen soubor 151 pacientů, kteří byli odoperováni pro diagnózu primární hyperparathyreózy. Byl proveden průzkum na onemocnění provázející primární hyperparathyreózu a možné následky v pooperačním období. Zvláštní kapitolu tvoří popis 10 případů v dětském věku. Před operací je snaha lokalizovat patologické příštítné tělísko. Předoperační lokalizační vyšetření se dají rozdělit na...Surgical management of primary hyperparathyroidism is a very effective method. The target is to cure primary hyperparathyroidism and to reach normal calcium levels. This results in an improvement of health condition and resolution or at least moderation of symptoms. Complications are infrequent and mortality is very low. Surgical management is definite, safe and effective. Number, location and surrounding structures of parathyroid glands with emphasis on parathyroidectomy were studied on 101 cadaverous bodies. We recommend the performance of parathyroidectomy in a circumscribed area 2 cm in diameter, 1 cm cranially to the intersection of the inferior thyroid artery and recurrent laryngeal nerve. We identified 75% of samples as parathyroid glands. The rest of collected samples were lymph nodes, fat particles and thyroid or lipothymic tissue. During the course of this study, we have tested techniques of miniinvazive procedures. In the following part, the bony, metabolic and biochemical syndromes are evaluated. We presented series of 151 patients that have been operated with the diagnosis of primary hyperparathyroidism.
