Cílem práce bylo zjistit účinnost antiagregační léčby u starých a polymorbidních pacientů. Zajímalo nás, zda vybrané laboratorní parametry, komorbidity a současně užívaná léčba mohou mít vliv na účinnost antiagregačního efektu malých dávek kyseliny acetylsalicylové. Ve vyšetřených souborech jsme prokázali vysoké hodnoty 11-dehydrotromboxanu B2 v moči, které svědčily o neúčinné antiagregační léčbě. V našem souboru její prevalence dosahovala 46 %. Při porovnání souborů aspirin senzitivních a rezistentních pacientů byly zjištěny statisticky významné rozdíly v hodnotách C reaktivního proteinu, údajích o kouření, překonané ischemické cévní mozkové příhodě a fibrilaci síní. Pacienti s účinnou antiagregační léčbou byli častěji léčeni statiny, nitráty a betablokátory. Vyšetřením sérových koncentrací salicylátů jsme zjistili, že na vysoké prevalenci neúčinné antiagregační léčby se významně podílela non-compliance pacientů. Práce upozornila, které klinické a laboratorní faktory i léčiva mohou ovlivňovat antiagregační léčbu kyselinou acetylosalicylovou a na které se je nutno zaměřit v dalších potřebných studiích. The aim of our study was to determine the efficacy of antiplatelet therapy in elderly and polymorbid patients. We were interested to know whether selected laboratory parameters, co-existing conditions, and concomitant therapy may affect the antiplatelet effect of low-dose acetylsalicylic acid. In our study groups, we demonstrated high urinary levels of 11-dehydrothromboxane B2 suggesting ineffective antiplatelet therapy. In our series, its prevalence was in excess of 46%. A comparison of groups of aspirin-sensitive and aspirin-resistant patients revealed statistically significant differences in CRP levels, smoking status, previous stroke, and atrial fibrillation. Patients with effective antiplatelet therapy were more often treated with statins, nitrates, and beta-blockers. Determination of the serum levels of salicylates showed a major role in the high prevalence of ineffective antiplatelet therapy was played by patient non-compliance.
