Řád blanokřídlí (Hymenoptera) čítá na sto tisíc druhů hmyzu (jen na našem území žije kolem 15 000 druhů) 1 . Někteří jedinci disponují bodavým a jedovým aparátem, který jim primárně slouží k obraně před nepřítelem nebo omráčení kořisti1, 2, 3. Bodnutí nebo kousnutí člověka se může manifestovat různým způsobem. Ne příliš častou, ale závažnou nebo dokonce fatální komplikací je systémová alergická reakce. Systémová imunoterapie alergeny příslušného hmyzu ("Specific Alergen Immunotherapy" - SAIT), zkráceně jedová imunoterapie ("Venom Immunotherapy" - VIT), je jedinou kauzální terapií, zabraňující vzniku závažné nebo život ohrožující IgE zprostředkované anafylaktické reakci při následujícím bodnutí příslušným druhem hmyzu4 . Jejím smyslem je dlouhodobé navození imunologické tolerance (specifické hyposenzibilizace) proti systémově podávaným alergenům5 . Iniciální pokusy o VIT byly prováděny již ve 20.letech minulého století, přičemž zprvu využívané celotělové extrakty ("whole body extreacts" - WBEs) byly, vzhledem ke srovnatelné účinnosti s placebem, nahrazeny v 70. letech extrakty jedovými, jež se užívají pro diagnostiku i terapii dodnes3 . Mechanismus navození imunologické tolerance prostřednictvím SAIT je komplexní a dosud ne zcela probádaný. Pravděpodobně navozuje tvorbu tzv. "blokujících" protilátek třídy...
