[cze][eng] Práce se zabývá sekundárním, tedy získaným cholesteatomem. Ten vzniká na podkladě retrakční kapsy. Proces je podmíněn tubární dysfunkcí, jež vede přes otitis media secretorica ke vzniku retrakční kapsy a dále může v některých případech dojít k rozvoji otitis media chronica s cholesteatomem. Cholesteatom je perlovitý útvar nacházejí se ve středoušní dutině s agresivním chováním vůči okolním strukturám. Může vést k závažným až život ohrožujícím komplikacím. Proto mají smysl opatření vedoucí k prevenci jeho vzniku. Patří sem sledování rizikových pacientů, správná léčba onemocnění předcházejících vznik získaného cholesteatomu a v neposlední řadě sanační operace nekontrolovatelné nebo jinak problematické retrakční kapsy, prováděné jako tympanotomie, attikotomie či attikoantrotomie dle rozsahu a lokalizace kapsy jako prevence vzniku cholesteatomu. Práce je doplněna o soubor pacientů operovaných na ORL klinice FNKV. Tento soubor má velké zastoupení pacientů s rozštěpovou vadou (44%). Ta se často pojí s tubární dysfunkcí a proto je tato skupina riziková a preventivní opatření jsou zde zcela na místě.The dissertation deals with acquired (secondary) cholesteatoma. It arises from a retraction pocket. The process is conditioned on tubal dysfunction that leads through secretory otitis media to the development of retraction pocket and then may result in chronic otitis media with cholesteatoma. Cholesteatoma is a pearl shaped formation located in the middle ear cavity with aggressive behavior to surrounding structures. It may lead to serious life-threatening complications. Therefore all measures preceding its formation are important. These measures concern: follow-up of risk patients, appropriate treatment preceding the development of acquired cholesteatoma or therapeutic operation of uncontrollable or problematic retraction pocket, performed as tympanotomy, atticotomy, or atticoantrotomy according to the size and localization of the pocket as a prevention of the formation of cholesteatoma. The dissertation is completed with a group of patients who underwent middle ear surgery at the ENT clinic of FNKV. This group of patients has a high proportion of patients with cleft (44%), often connected with tubal dysfunction and so they represent a risk group and preventive measures are justified.
