Cíl studie: posoudit možnosti detekce volné DNA v plazmě pacientů po transplantaci srdce a využití teto metody za účelem diagnostiky akutní rejekce srdečního štěpu. Metodika: Pomocí PCR jsme analyzovali přítomnost různých úseků DNA v plazmě u pacientů po transplantaci srdce. Výsledky: Na několika DNA markerech jsme prokázali, že citlivost PCR analýzy není dostatečná (detekce ředění maximálně okolo 1:50) pro průkaz nízké koncentrace dárcovské DNA v plazmě příjemce. Také jsme prokázali cca 40% přítomnost Y chimerismu v ženských tkáních - to dále snižuje možnost použití PCR pro specifickou detekci volné DNA. Nicméně, použití nested PCR umožní detekci volné DNA (v případě analýz nekompetujících úseků DNA) dárcovského původu i při ředění 1:1000000. Závěr: Vzhledem k nízké citlivosti PCR pro danou aplikaci a vysoké míře chimerismu je nepravděpodobné, že by detekce pomocí volné DNA mohla být v blízké budoucnosti použita k rutinnímu vyšetření akutní rejekce. Aim of the study: To evaluate the possibility of the detection of free DNA (like a marker of acute rejection) in the plasma of patients after heart transplantation. Methods: Using PCR different genetic markers were analyzed in plasma of the patients after heart transplantation. Results: On more genetic markers we have shown, that the PCR sensitivity is not high enough (detection of the dilution max ~ 1:50) to prove the low concentration of free plasma DNA of donor origin in the case, that forensic markers or dialelic polymorphisms are used. In the tissue of female aortas, we have detected the presence of Y chromosome specific DNA in ~ 40%. Conclusions: Because of the low sensitivity of the method used and high prevalence of chimerism, it is not likely, that in the near future could be the detection of free plasma DNA used for routine examination of acute rejection.
