[cze][eng] Diabete mellitus 1. typu patří mezi autoimunitní onemocnění, které postihuje především děti a mladé lidi. Při tomto onemocnění dochází k destrukci B-buněk Langerhansových ostrůvků pankreatu, které jsou zodpovědné za sekreci inzulínu. Jeho nedostatek pak vede k poruchám metabolismu sacharidů a jiných živin. K destrukci B-buněk dochází postupně a první klinické příznaky se objeví až při zničení určitého procenta B-buněk pankreatu. Po propuknutí obtíží se nemocný stává doživotně závislý na exogenním podávání inzulínu. Incidence diabetes mellitus 1. typu celosvětově stoupá a jsou patrné značné geografické rozdíly mezi jednotlivými zeměmi. To nám napovídá, že v patogenezi onemocnění mají nepochybně velký vliv enviromentální faktory. Významnou roli hrají i faktory genetické a imunologické mechanizmy. V mé práci se snažím nastínit možnosti prevence a s ní související predikce diabetes mellitus 1. typu. Popisuji především genetické a imunologické markery, které lze použít k předpovědi genetického rizika a ke sledování průběhu období před manifestací onemocnění. Zmiňuji se i o možnostech primární prevence, která je však v současné době zatím pouze na experimentálním poli. V rámci sekundární prevence popisuji některé léčebné postupy, které jsou již běžně používané ve světě i v České republice.
